上海市华漕社区341名产妇产后抑郁及影响因素分析

摘　　要：目的了解华漕社区产妇产后抑郁状况及影响因素,为产后抑郁高危人群预防提供科学依据。方法选取本社区2019-01-01-2019-12-31登记管理并分娩的341名产妇为调查对象,调查和分析产后抑郁状况及其影响因素。结果341名产妇中有58名产后抑郁,产后抑郁发生率为17.01%。单因素分析结果显示,产妇职业状态(χ^(2)=3.990,P=0.046)、配偶职业状态(χ^(2)=5.072,P=0.024)和产妇文化程度(χ^(2)=6.898,P=0.032)是影响产后抑郁的主要因素;多因素Logistic回归分析结果显示,产妇爱人不在职(χ^(2)=5.550,P=0.018)和产妇文化程度在初中及以下(χ^(2)=4.192,P=0.041)是产后抑郁的主要影响因素。结论华漕社区产妇产后抑郁水平较低,社区产后随访医生应加强产妇心理相关知识培训,重点关注夫妻双方不在职、文化程度相对较低的产妇,并进行有针对性的心理疏导和干预,使其保持身心健康,家庭和睦。Objective To investigate the related risk factors of postpartum depression,providing scientific basis for prevention of high-risk populations of postpartum depression.Methods A total of 341 cases who registered and childbirth in our community from January 1,2019 to December 31,2020 were enrolled and analyzed for the status of postpartum depression and its related risk factors.Results The occurrence of postpartum depression was 17.01%(58/341).Single factor analysis results showed that maternal occupation status(χ^(2)=3.990,P=0.046),spouse occupational status(χ^(2)=5.072,P=0.024)and maternal education(χ^(2)=6.898,P=0.032)were the main postpartum depression factors;multi-factors Logistic regression analysis results showed that maternal love was not in-service(χ^(2)=5.550,P=0.018)and maternal education level in junior high school and below(χ^(2)=4.192,P=0.041)were the main factors of postpartum depression.Conclusion The level of postpartum depression is low in Huacao community,so the community postpartum follow-up doctors should strengthen the training of maternal psychological knowledge,focus on the postpartum women who are not employed by the husband and wife and have relatively low educational level,and carry out targeted psychological counseling and intervention to keep them physically and mentally healthy and family harmony.
