髂动脉分支支架植入重建髂内动脉临床结果分析

摘　　要：目的探讨髂总动脉瘤腔内治疗过程中利用髂动脉分支支架(IBD)保留髂内动脉的临床结果及近期疗效。方法回顾性分析2016年6月至2020年9月南京医科大学第一附属医院血管外科使用IBD技术重建髂内动脉的14例患者的临床资料。本组14例患者中腹主动脉瘤合并髂动脉瘤4例,腹主动脉瘤合并髂总动脉瘤和髂内动脉瘤2例,孤立性髂动脉瘤7例,腹主动脉夹层合并髂动脉瘤1例。髂总动脉瘤直径为(32.10±9.05)mm。结果植入自制IBD和临床试验支架各7例。技术成功率为92.9%(13/14)。术后中位随访时间6个月(范围1~42个月)。随访期内1例因髂总动脉瘤破裂而死亡,其余病例植入的IBD支架均通畅,瘤体无扩张。结论IBD是髂动脉瘤腔内治疗中保留髂内动脉安全而有效的方法。Objective To investigate the clinical results and short-term efficacy of implanting iliac branched device(IBD)for preserving the internal iliac artery during the endovascular treatment of iliac artery aneurysms.Methods A retrospective analysis was performed on the clinical data of 14 patients treated with IBD technique to reconstruct the internal iliac artery at the Department of Vascular Surgery of the First Affiliated Hospital of Nanjing Medical University from June 2016 to September 2020.Among the 14 patients,there were 4 cases of abdominal aortic aneurysm with iliac artery aneurysm,2 cases of abdominal aortic aneurysm with common and internal iliac artery aneurysm,7 cases of isolated iliac artery aneurysm and 1 case of abdominal aortic dissection with iliac artery aneurysm.The diameter of the common iliac artery aneurysms was(32.10±9.05)mm.Results Seven cases of physician-modified IBD and 7 cases of clinical trial stents were implanted.The technical success rate was 92.9%(13/14).The mean follow-up time was 6 months.During the follow-up,one death occurred due to the rupture of the common iliac artery aneurysm while for the remaining cases,the implanted IBDs were patent without enlargement of aneurysm.Conclusion IBD is a safe and effective method for preserving the internal iliac artery in the endovascular treatment of iliac artery aneurysms.
