Turbo Hawk斑块旋切术联合药物涂层球囊治疗长段股浅动脉硬化闭塞症的疗效分析

摘　　要：目的探讨Turbo Hawk斑块旋切术联合药物涂层球囊(DCB)治疗股浅动脉长段硬化闭塞症的临床疗效。方法回顾性分析连云港市第二人民医院血管外科2017年1月至2019年6月采用Turbo Hawk旋切术联合DCB治疗的21例股浅动脉长段硬化闭塞症患者的临床资料。所有患者均处理单侧病变。其中男性15例,女性6例,年龄(71.3±6.5)岁。记录患者患肢缺血症状、技术成功率、靶血管一期通畅率、踝肱指数(ABI)、Rutherford分级、最小管腔直径(MLD)和术中、术后并发症情况,并同患者术前资料进行统计分析。结果所有患者术后患肢缺血症状得到不同程度改善,技术成功率为95.24%(20/21)。21例患者均获得随访,随访时间为(12.5±5.7)个月。靶血管一期通畅率为71.43%(15/21)。术后6个月,ABI与术前相比明显升高(0.87±0.16比0.38±0.11),差异有统计学意义(t=14.026,P<0.001)。MLD值较术前增加(3.23±0.48比0.54±0.18),差异有统计学意义(t=26.705,P<0.001)。Rutherford分级与术前比较明显改善。随访期间未发现重大不良事件。结论Turbo Hawk斑块旋切术联合DCB治疗股浅动脉长段硬化闭塞症安全可靠,短期疗效良好。Objective To explore the clinical efficacy of Turbo Hawk atherectomy combined with drug-coated balloon(DCB)in the treatment of long-segment superficial femoral artery occlusive disease.Methods A retrospective study was performed on the clinical data of 21 patients with long-segment superficial femoral artery occlusive disease treated with Turbo Hawk atherectomy combined with DCB admitted to the Department of Vascular Surgery of the Second People's Hospital of Lianyungang from January 2017 to June 2019.All the patients were treated with unilateral lower limb vascular disease,among which there were 15 males and 6 females,with an age of(71.3±6.5)years.Patients'affected limb ischemia symptoms,technical success rate,primary patency rate of target vessels,ankle brachial index(ABI),Rutherford classification,minimal lumen diameter(MLD),and intraoperative and postoperative complications were recorded and statistically analyzed with the patients'preoperative data.Results The postoperative ischemic symptoms of the affected limbs of all the patients were all relieved to varying degrees,with a technical success rate of 95.24%(20/21).All the patients were followed up for(12.5±5.7)months.And a primary patency rate of target vessels of 71.43%(15/21).The postoperative ABI was significantly higher than that before the operation(0.87±0.16 vs.0.38±0.11),and the difference was statistically significant(t=14.026,P<0.001).The postoperative MLD was significantly higher than that before the operation(3.23±0.48 vs.0.54±0.18),and the difference was statistically significant(t=26.705,P<0.001).The postoperative Rutherford classification was significantly improved compared with that before the operation.There was no serious adverse event during the follow-up period.Conclusion Turbo Hawk atherectomy combined with DCB is an ideal option for the treatment of long-segment superficial femoral artery occlusive disease,which is safe and reliable with good short-term efficacy.
