摘  要:目的探究血清微小RNA-155(miR-155)、程序性细胞死亡因子4(PDCD4)水平与糖尿病肾病(DN)患者血管钙化的相关性。方法选取2019年1月-2020年8月在本院治疗的未透析DN患者113例为研究对象,根据冠状动脉是否合并血管钙化分为合并血管钙化组(钙化组) 52例和非血管钙化组(非钙化组) 61例,比较2组患者一般资料,利用贝克曼全自动生化分析仪检测血液指标,利用荧光定量PCR技术检测血清miR-155、PDCD4 mRNA水平。Pearson法分析血管钙化组患者血清miR-155和PDCD4 mRNA的相关性,多因素Logistic分析影响DN患者血管钙化的危险因素。结果钙化组miR-155及血钙水平明显低于非钙化组(P <0.05),PDCD4 mRNA、血磷、甲状旁腺激素(PTH)、尿素氮(BUN)、血清肌酐(SCr)水平明显高于非钙化组(P <0.05);与钙化不明显组相比,中度、重度钙化组患者血清miR-155水平依次降低,PDCD4 mRNA水平依次升高,上述差异均有统计学意义(P <0.05);血管钙化组患者血清miR-155、PDCD4 mRNA水平呈显著负相关(r=-0.672,P=0.000);多因素Logistic回归分析,结果显示PTH水平高、miR-155水平低、PDCD4 mRNA水平高均是影响DN患者血管钙化的危险因素(P <0.05)。结论 DN合并血管钙化患者血清miR-155表达下降,PDCD4表达升高,且与血管钙化的严重程度有关,可能参与DN患者血管钙化的发生发展。Objective To explore the relationship between serum microRNA-155( miR-155),programmed cell death factor4( PDCD4) and vascular calcification in patients with diabetic nephropathy( DN). Methods A total of 113 patients with non dialysis( DN) who were treated in our hospital from January 2019 to August 2020 were selected as the research objects. According to whether the coronary artery was complicated with vascular calcification,they were divided into vascular calcification group( calcification group,52 cases) and non vascular calcification group( non calcification group,61 cases),the general data of the two groups were compared,blood indexes were detected by Beckman automatic biochemical analyzer,and levels of serum miR-155 and PDCD4 mRNA were detected by fluorescence quantitative PCR. Pearson method was used to analyze the correlation between serum miR-155 and PDCD4 mRNA in patients with vascular calcification,and multivariate Logistic analysis was used to analyze the risk factors of vascular calcification in DN patients. Results The levels of miR-155 and serum calcium in calcification group were significantly lower than those in non calcification group( P < 0. 05),the levels of PDCD4 mRNA,blood phosphorus,parathyroid hormone( PTH),urea nitrogen( BUN) and serum creatinine( SCR) in calcification group were significantly higher than those in non calcification group( P < 0. 05);compared with those in the non calcification group,the level of serum miR-155 in moderate and severe calcification groups decreased in turn,and the level of PDCD4 mRNA increased in turn,with the above differences statistically significant( P < 0. 05);the levels of miR-155 and PDCD4 mRNA in vascular calcification group were negatively correlated( r =-0. 672,P = 0. 000);multivariate Logistic regression analysis showed that high PTH level,low miR-155 level and high PDCD4 mRNA level were risk factors of vascular calcification in DN patients( P <0. 05).
