摘 要:目的分析急性非结石性胆囊炎患者病原菌分布及耐药情况,为合理选用抗菌药物提供科学依据。方法回顾性分析本院2015年2月-2020年6月共82例急性非结石性胆囊炎患者的临床资料。结果共送检118份标本,80份检出病原菌,阳性率为67.8%,共检出病原菌62株;其中革兰阴性菌48株(77.4%),革兰阳性菌14株(22.6%);前4位的病原菌为大肠埃希菌32株(51.6%)、肺炎克雷伯菌9株(14.5%)、屎肠球菌8株(12.9%)、铜绿假单胞菌3株(4.8%);大肠埃希菌、肺炎克雷伯菌对头孢唑林、头孢呋辛的耐药率较高,对美罗培南及亚胺培南无耐药;屎肠球菌对大多数常用抗菌药物的耐药率均较高,对万古霉素和替考拉宁无耐药。结论本院引起急性非结石性胆囊炎患者的病原菌以革兰阴性杆菌为主,应根据药敏试验结果合理选用抗菌药物。Objective To study the distribution and antimicrobial resistance of pathogens causing acute acalculous cholecystitis,so as to provide scientific evidence for the reasonable use of antibiotics. Methods The clinical data collected from a total of 82 patients with acute acalculous cholecystitis was analyzed retrospectively in the hospital from February 2015 to June 2020.Results A total of 118 specimens were tested,the pathogens was detected in 80 of them with the positive rate of 67. 8%,and a total of 62 strains were detected;48 strains of gram-negative bacilli and 14 strains of gram-positive cocci were detected,accounting for 77. 4% and 22. 6%;the top 4 species were 32 strains of Escherichia coli,9 strains of Klebsiella pneumoniae,8 strains of Enterococcus faecium and 3 strains of Pseudomonas aeruginosa,accounting for 51. 6%,14. 5%,12. 9%,and4. 8% respectively;Escherichia coli and Klebsiella pneumoniae showed higher drug resistance rates to cephazolin and cefuroxime;were both zero to meropenem and imipenem;the drug resistance rates of Enterococcus faecium were higher to the most of commonly used antibacterial drugs;were both zero to vancomycin and teicoplanin. Conclusion The pathogens causing acute acalculous cholecystitis were mainly gram-negative bacilli,so medical staff should choose reasonably antibacterial drugs according to antimicrobial susceptibility test results.
