摘 要:目的了解海南结核病患者利福平rpo B耐药基因突变位点流行病学特征。方法收集2014年-2019年海南疑似结核病患者利福平耐药基因检测结果。计数资料采用频数和百分比进行统计描述,计量资料采用均数±标准差进行统计描述,组间差异性比较采用卡方检验或Fisher确切概率法,以P <0.05为差异有统计学意义。结果共检测到8类利福平耐药突变类型,其中最常见的耐药突变位点为rpo B531位点。随着时间推移,出现某些新型突变位点,如513CAA→CCA+516GAC→GTC联合突变位点。海口市、儋州市、文昌市和万宁市为耐药突变位点最常见的4个地区,分别占所有突变位点的19.5%、12.5%、10.0%和9.4%。此外,不同人群耐药突变位点有差异。结论结核病患者利福平耐药基因突变在不同时间、不同地区和不同人群中分布存在差异性。Objective To explore the epidemiological characteristics of rifampicin resistant mutations in tuberculosis patients in Hainan Province,China.Methods The information about rifampicin resistant mutations of suspected tuberculosis patients in Hainan from 2014 to 2019 was collected.Count data were described by frequency(percentage),and measurement data were described by mean±standard deviation respectively.The differences between different groups were compared by chi-square test or Fisher's exact probability.P<0.05 was considered statistically significant.Results Eight types of rifampicin resistant mutations were detected,among which rpob531 was the most common mutation.Over time,some new mutations appeared,such as 513CAA→CCA+516GAC→GTC mutation.Haikou,Danzhou,Wenchang and Wanning were the cities with the most rifampicin resistant mutations,accounting for 19.5%,12.5%,10.0%and 9.4%of all mutations,respectively.In addition,the mutations were different in different populations.Conclusion There were differences in the distribution of rifampicin resistant mutations in different time,different regions and different populations.
