经鼻高流量湿化氧疗仪在COPD患者中的临床疗效分析

摘　　要：目的探讨经鼻高流量湿化氧疗仪在COPD患者中的临床疗效分析。方法取自泉州市第一医院呼吸内科2020年2月—2021年2月间收治的慢性阻塞性肺疾病(chronic obstructive pulmonary disease,COPD)患者103例根据入院顺序单双号予以分组,其中单号对照组51例患者采用普通湿化瓶氧疗,双号研究组52例患者采用经鼻高流量湿化氧疗方案,对其临床疗效进行观察,并对其应用价值深入探讨。结果研究组患者的再次插管率5.77%低于对照组患者的再次插管率23.53%,呼衰发生率为1.92%,比对照组患者的呼衰发生率17.65%要低,且研究组患者的肺部感染评分、呼吸频率、心率、二氧化碳分压均等疗效指标均低于对照组患者,氧分压、pH值等指标则高于对照组患者,组间对比差异有统计学意义(P<0.05)。结论经鼻高流量湿化氧疗在COPD患者中的临床疗效值得肯定,能够降低不良事件,且各项呼吸功能指标明显改善,应用价值较高。Objective To investigate the clinical efficacy of nasal high-flow humidification oxygen therapy in patients with COPD.Methods 103 patients with chronic obstructive pulmonary disease(COPD)admitted from February 2020 to February 2021 in the department of respiratory medicine of Quanzhou First Hospital were divided into groups according to the order of admission,odd and even numbers,among them,51 single patients in control group were treated with ordinary humidifying bottle oxygen therapy,and 52 double patients in study group were treated with nasal high-flow humidifying oxygen therapy.Observed its clinical efficacy and discussed its application value in depth.Results The reintubation rate of the study group was 5.77%lower than that of the control group(23.53%),and the incidence of aspiration failure was 1.92%lower than that of the control group(17.65%),the pulmonary infection score,respiratory frequency,heart rate and carbon dioxide pressure in the study group were lower than those in the control group,while the oxygen pressure and pH value were higher than those in the control group,there was statistical difference between the two groups(P<0.05).Conclusion Nasal high-flow humidification oxygen therapy in patients with COPD clinical efficacy is positive,can reduce adverse events,and improve the respiratory function index significantly,with higher application value.
