戈舍瑞林联合腹腔镜治疗卵巢型子宫内膜异位症的临床价值

摘　　要：目的探讨戈舍瑞林联合腹腔镜治疗卵巢型子宫内膜异位症的临床价值。方法选择2018年4月—2019年10月医院收治的卵巢型子宫内膜异位症患者80例,按照随机数字表法分为两组,各40例。对照组在全身麻醉下实施腹腔镜手术干预,观察组则于对照组基础上给予戈舍瑞林,比较两组干预前后抗苗勒管激素水平,比较两组干预前后卵巢超声指标,分析随访期间疼痛NRS评分变化情况,随访1年,比较两组受孕情况。结果干预1年后,两组抗苗勒管激素水平均显著高于干预前(P<0.05),且观察组抗苗勒管激素水平高于对照组(P<0.05),干预1年后,观察组卵巢超声指标中排卵期窦卵泡数(average antral follicle count,AFC)水平高于对照组(P<0.05),卵巢基质动脉搏动指数(pulsatility i n dex,PI)和卵巢动脉阻力指数(resistance index,RI)均显著小于对照组(P<0.05),干预3个月后和干预1年后,观察组疼痛NRS评分低于对照组(P<0.05),观察组自然受孕比例显著高于对照组(P<0.05)。结论针对卵巢型子宫内膜异位症,应用戈舍瑞林联合腹腔镜治疗,能有效的调节卵巢功能,缓解疼痛症状,提高自然妊娠几率。Objective To explore the clinical value of goserelin combined with laparoscopic in the treatment of ovarian endometriosis.Methods 80 cases with ovarian endometriosis admitted to our hospital from April 2018 to October 2019 were randomly divided into two groups according to random number method,each with 40 cases.The control group was given laparoscopic surgery intervention under general anesthesia,while the observation group was given goserelin on the basis of the control group.The levels of anti-Mullerian hormone before and after intervention were compared between the two groups.The ovarian ultrasound indexes before and after intervention were compared.The changes of pain NRS score during follow-up were analyzed.The pregnancy status of the two groups was compared after one year of follow-up.Results After one year’s intervention,the levels of anti-Mullerian hormone in both groups were significantly higher than those before intervention(P<0.05),and the levels of anti-Mullerian hormone in observation group were higher than those in control group(P<0.05).After 1 year of intervention,the average antral follicle count(AFC)level in the ovulation ultrasound index of the observation group was higher than that of the control group(P<0.05),ovarian matrix artery pulsatility index(PI)and resistance index(RI)was significantly lower than that of the control group(P<0.05).The pain NRS score of the observation group was lower than that of the control group 3 months and 1 year after the intervention(P<0.05),and the natural conception rate of the observation group was significant higher than the control group(P<0.05).Conclusion For ovarian endometriosis,goserelin combined with laparoscopy can effectively regulate ovarian function,relieve pain symptoms and improve the probability of natural pregnancy.
