围产期孕妇生殖道感染的影响因素分析

摘　　要：目的探讨围产期孕妇生殖道感染的影响因素,为围产期孕产妇的管理提供指导。方法选取2017年1月1日-2019年12月31日在某院行产前检查及分娩的孕妇360例,均行阴道分泌物病原体培养,同时收集孕妇临床资料,采用Logistic回归分析生殖道感染危险因素。结果360例孕妇中共检出病原体感染者68例,病原体检出率为18.89%,其中B族链球菌、支原体和念珠菌感染率分别为7.22%、6.11%和5.56%;≥35岁、体重指数≥25kg/m^(2)、有流产史、有妊娠期糖尿病、有孕前糖尿病是生殖道感染的相关因素(P<0.05);Logistic回归分析结果显示年龄、体重指数、妊娠期糖尿病和孕前糖尿病是围产期孕妇生殖道感染的危险因素(OR=1.562、1.655、2.683和2.748,P<0.05)。结论围产期孕妇生殖道感染与孕妇年龄、体重指数、妊娠期糖尿病和孕前糖尿病有一定关系,在妊娠期管理中应给与足够关注。Objectives To investigate influence factors of genital tract infection in perinatal pregnant women provide guidance for clinical management of perinatal infection.Methods A total of 360 pregnant women who underwent prenatal examination and delivery in a hospital from January 1st,2017 to December 31st,2019 were selected.Vaginal secretions were cultured for pathogens.Clinical data of pregnant women were collected and risk factors of reproductive tract infection were analyzed by Logistic regression.Results A total of 68 cases of pathogen infection were detected among 360 pregnant women,and the pathogen detection rate was 18.89%.The infection rates of group B streptococcus,mycoplasma and Candida were 7.22%,6.11% and 5.56%,respectively.≥35 years old,body mass index≥25kg/m2,history of abortion,gestational diabetes,pre-pregnancy diabetes were the related factors of reproductive tract infection(P<0.05);Logistic regression analysis showed that age,body mass index,gestational diabetes mellitus and pre-pregnancy diabetes mellitus were risk factors of reproductive tract infection in perinatal pregnant women(OR=1.562,1.655,2.683 and 2.748,P<0.05).Conclusions Perinatal reproductive tract infection in pregnant women was related to maternal age,body mass index,gestational diabetes and pre-pregnancy diabetes,which should be paid enough attention in the management of pregnancy.
