摘 要:目的:探讨宫颈病变不同病理级别中证型为湿热证的分布特征及其与miR-10b表达的相关性。方法:将全部宫颈上皮内瘤变(CIN)和早期宫颈癌的患者根据病理级别分为低级别鳞状上皮内病变组(LSIL组,71例)、高级别鳞状上皮内病变组(HSIL组,45例)和早期宫颈癌组(23例)。对所有患者行中医证候判定,比较不同病理级别中湿热证的分布情况;对辨证为湿热证的患者进行中医证候评分,比较不同病理级别中湿热证患者的中医证候积分。采用实时荧光定量PCR检测法测定患者宫颈病变组织中的miR-10b含量,比较不同病理级别患者miR-10b含量差异;根据有无湿热证见症将患者分为湿热证组和非湿热证组,比较两组患者宫颈病变组织中miR-10b含量差异。结果:早期宫颈癌组中湿热证的构成比高于HSIL组(P<0.001),HSIL组高于LSIL组(P<0.001);早期宫颈癌组中湿热证的中医证候积分高于HSIL组(P<0.01),HSIL组高于LSIL组(P<0.01);早期宫颈癌组miR-10b表达量低于HSIL组(P<0.05),HSIL组低于LSIL组(P<0.001);湿热证组miR-10b相对表达量低于非湿热证组(P<0.05)。结论:湿热是CIN最常见的证候要素,也是促进宫颈病变进展的主要病理因素,抑制miR-10b表达可能是湿热促进CIN病变进展的机制之一。Objective:To investigate the distribution characteristics of damp-heat syndrome in different pathological grades of cervical lesions and its correlation with miR-10b expression.Methods:The patients with cervical intraepithelial neoplasia(CIN)and early cervical cancer were divided into the LSIL group(n=71),the HSIL group(n=45)and the early cervical cancer group(n=23)according to the pathological grades of cervical lesions.The patients were diagnosed with TCM syndrome,the distribution of damp-heat syndrome was compared in different pathological grades of cervical lesions.The patients with damp-heat syndrome were assessed by TCM syndrome integral,and TCM syndrome integral was compared in different pathological grades of cervical lesions.The expression of miR-10b in cervical tissues was detected by RT-qPCR and compared among the three groups.The distribution characteristics of damp-heat syndrome and its relationship with the expression of miR-10b were analyzed in the groups.Results:In terms of the ratio of damp-heat syndrome,it was higher in the early cervical cancer group than in the HSIL group(P<0.001),and it was also higher in the HSIL group than in the LSIL group(P<0.001).In terms of the TCM syndrome integral,it was higher in the early cervical cancer group than in the HSIL group(P<0.01),and it was also higher in the HSIL group than in the LSIL group(P<0.01).In terms of the expression of miR-10b,it was lower in the early cervical cancer group than in the HSIL group(P<0.05),and it was also lower in the HSIL group than in the LSIL group(P<0.001);it was lower in the damp-heat syndrome group than non damp-heat syndrome group(P<0.05).Conclusion:Damp-heat is the most common syndrome element of CIN,which is the main pathological factor promoting the progression of cervical lesions.The inhibition of miR-10b expression may be one of the mechanisms of damp-heat promoting the progression of cervical lesions.
