摘　　要：目的探讨直接分离法建立后腹膜腔在泌尿外科后腹腔镜手术中的应用效果及安全性。方法选择2019年1月至2021年1月我院泌尿外科行后腹腔镜手术的70例患者,随机分为A组(自制气囊法)、B组(手指分离法)、C组(直接分离法)。对比三组的效果及安全性。结果三组患者中,A组出现3例腹膜破裂,B组出现1例腹膜破裂,其余患者均顺利建立后腹膜腔。术后随访显示,患者的Trocar切口愈合良好。三组的建腔时间比较差异有统计学意义(P<0.05)。A组的建腔时间长于B组、C组,B组的建腔时间长于C组(P<0.05)。三组的建腔失血量及皮下气肿、腹腔破裂发生率比较差异无统计学意义(P>0.05)。结论直接分离法建立后腹膜腔的建腔时间短,操作简便,并发症少,适宜在泌尿外科后腹腔镜手术中推广应用。Objective To analyze the application effect and safety of direct separation method in the establishment of retroperitoneal cavity during retroperitoneal laparoscopic surgery in department of urology.Methods 70 patients undergoing retroperitoneal laparoscopic surgery in our hospital from January 2019 to January 2021 were randomly divided into group A(self-made balloon method),group B(finger separation method)and group C(direct separation method).The effects and safety of the three groups were compared.Results Among the three groups,3 cases in group A and 1 case in group B had peritoneal rupture,and the retroperitoneal cavity was successfully established in the other patients.Postoperative follow-up showed that the Trocar incision healed well.Statistical difference was found in the cavity formation time among the three groups(P<0.05).The cavity formation time of group A was longer than those of group B and group C,and the cavity formation time of group B was longer than that of group C(P<0.05).No statistical difference was found in the cavity formation blood loss,and the incidences of subcutaneous emphysema and abdominal rupture among the three groups(P>0.05).Conclusions Direct separation method in the establishment of retroperitoneal cavity has shorter cavity formation time,simple operation and few complications,which is suitable for promotion and application in retroperitoneal laparoscopic surgery in department of urology.
