IL-6、PCT联合检验在儿童脓毒症病情和预后评估中的应用价值

摘　　要：目的探讨与分析白细胞介素6(IL-6)、降钙素原(PCT)联合检验在儿童脓毒症病情和预后评估中的应用价值。方法回顾性分析2019年2月至2020年8月我院收治的96例脓毒症患儿的临床资料,比较不同病情严重程度、生存状况脓毒症患儿的血清IL-6和PCT水平,并分析IL-6、PCT单一和联合检验判断脓毒症患儿病情和预后的临床价值。结果脓毒症休克组的IL-6、PCT水平高于严重脓毒症组、脓毒症组,严重脓毒症组的IL-6、PCT水平高于脓毒症组(P<0.05)。死亡组的IL-6、PCT水平高于生存组(P<0.05)。IL-6、PCT联合检验预测脓毒症患儿病情的曲线下面积(AUC)为0.876、特异度为83.21%、敏感度为82.48%,预测预后的AUC为0.819、特异度为75.32%、敏感度为81.96%,均高于单项检验。结论IL-6、PCT均是判断脓毒症患儿病情和评估预后的有效指标,联合检验对脓毒症患儿病情预测和预后评估价值更高。Objective To explore and analyze the application value of combined test of interleukin 6(IL-6)and procalcitonin(PCT)in the evaluation of disease condition and prognosis of children with sepsis.Methods The clinical data of 96 children with sepsis admitted to our hospital from February 2019 to August 2020 were retrospectively analyzed.The serum IL-6 and PCT levels of sepsis children with different severity and survival status were compared,and the clinical value of single and combined tests of IL-6 and PCT in judging the disease condition and prognosis of sepsis children was analyzed.Results The IL-6 and PCT levels of the septic shock group were higher than those of the severe sepsis group and the sepsis group,the IL-6 and PCT levels of the severe sepsis group were higher than those of the sepsis group(P<0.05).The IL-6 and PCT levels of the death group were higher than those of the survival group(P<0.05).The area under curve(AUC)of combined detection of IL-6 and PCT in sepsis children was 0.876,the specificity was 83.21%,and the sensitivity was 82.48%;The AUC for predicting prognosis was 0.819,the specificity was 75.32%,and the sensitivity was 81.96%;All were higher than single tests.Conclusions Both IL-6 and PCT are effective indicators for judging the disease condition of children with sepsis and evaluating the prognosis.The combined detection is of high value for disease condition prediction and prognosis assessment.
