马来酸左旋氨氯地平与苯磺酸氨氯地平对轻中度H型高血压患者的疗效及安全性比较

摘　　要：目的对比马来酸左旋氨氯地平与苯磺酸氨氯地平治疗轻中度H型高血压的临床效果。方法从LEADER研究入选的患者中筛选出轻中度H型高血压患者,根据患者使用降压药物的情况分为苯磺酸氨氯地平组(n=134)和马来酸左旋氨氯地平组(n=159)。比较两组治疗前后的收缩压、舒张压,以及两组的主要不良心脑血管事件(major adverse cardiovascular and cerebrovascular events,MACCE)、不良反应的发生情况。结果治疗前,苯磺酸氨氯地平组舒张压高于马来酸左旋氨氯地平组(P<0.05),但两组收缩压比较,差异无统计学意义(P>0.05)。治疗12及24个月后,两组收缩压及舒张压均较治疗前下降(P<0.01)。治疗后,苯磺酸氨氯地平组舒张压高于马来酸左旋氨氯地平组(P<0.05),但两组收缩压比较,差异无统计学意义(P>0.05)。两组治疗12及24个月时总有效率、MACCE及不良反应发生率比较,差异均无统计学意义(P>0.05)。结论马来酸左旋氨氯地平与苯磺酸氨氯地平对轻中度H型高血压均具平稳、长效的降压作用,且安全性相似。Objective To compare the clinical effects of L-evoamlodipine maleate and amlodipine besylate in the treatment of mild to moderate H-type hypertension.Methods Patients with mild to moderate H-type hypertension were selected from the patients enrolled in the LEADER study,and were divided into amlodipine besylate group(n=134)and L-evoamlodipine maleate group(n=159)according to the situation of antihypertensive drugs.The systolic blood pressure,diastolic blood pressure,and major adverse cardiovascular and cerebrovascular events(MACCE)and adverse reactions in both groups were compared before and after treatment.Results Before treatment,the diastolic blood pressure in amlodipine besylate group was higher than that in L-amlodipine maleate group(P<0.05).However,there was no significant difference in systolic blood pressure between the two groups(P>0.05).after 12 and 24 months of treatment,the systolic blood pressure and diastolic blood pressure in both groups decreased compared with before treatment(P<0.01).After treatment,the diastolic blood pressure in amlodipine besylate group was higher than that in L-amlodipine maleate group(P<0.05),but there was no significant difference in systolic blood pressure between the two groups(P>0.05).There was no significant difference in the total effective rate,MACCE and incidence of adverse reactions between the two groups at 12 and 24 months of treatment(P>0.05).Conclusion Both L-evoamlodipine maleate and amlodipine benzenesulfonate have a stable and long-term antihypertensive effect on mild to moderate H-type hypertension with similar safety.
