干扰素在中国人尖锐湿疣治疗中的应用及系统评价(荟萃分析)

摘　　要：干扰素因其免疫调节、抗增殖和抗病毒的特性,用于治疗尖锐湿疣多年。但国内关于干扰素治疗尖锐湿疣的系统性评价性文章很少。本文旨在对干扰素治疗尖锐湿疣的疗效性文献进行系统分析。我们以中国知网、万方、维普数据库作为搜索来源,经2位评估者在相应的纳入和排除标准下独立搜索、筛选相应研究,最终共纳入17篇中文文献(2000年1月—2021年6月)。进一步提取其基本特征和相关指标进行分析。结果表明,国内干扰素治疗尖锐湿疣的方法主要为联合局部药物和物理治疗。干扰素的使用方式包括针剂局部皮损内注射和凝胶、软膏栓剂类外用。与仅用传统物理治疗尖锐湿疣相比,联合干扰素治疗,无论是注射干扰素还是局部外用干扰素,均取得较好疗效,临床有效率显著上升(P<0.05);临床疗效因治疗剂量增加而增加;复发率显著下降(P<0.05)。此外,文献回顾分析显示,干扰素治疗并不增加不良反应发生率(P>0.05)。本研究表明,干扰素在我国尖锐湿疣治疗中主要为联合用药,可以降低复发率、提高临床有效率。Interferon has been used for the treatment of condyloma acuminatum for many years due to its immunomodulatory,anti-proliferative and anti-viral properties.At present however,there are few systematic review articles on interferon treatment for the condyloma acuminatum.This article aims to analyze the literature systematically on the efficacy of interferon in the treatment of condyloma acuminatum in China.We used China National Knowledge Infrastructure(CNKI),Wanfang,VIP database as well as reference lists of relevant studies as the search sources.Two reviewers independently screened searched studies,extracted data and evaluated their methodological qualities.A total of 17 Chinese articles were finally included(January 2000-June 2021).The results show that in China,interferon treatment for condyloma acuminatum is usually combined therapy with topical drugs and CO_(2) laser treatment.The ways of using interferon are:Interferon intralesional injection and gels,ointments for topical use.Compared with the CO2 laser treatment alone for condyloma acuminatum,the effective rate of combined interferon treatment was significantly increased(P<0.05),and the efficacy increased with the increasing dose.Meanwhile,the recurrence rate of condyloma acuminatum was significantly decreased(P<0.05).In addition,the reported adverse events of interferon did not show increased.(P>0.05).This study shows that interferon tends to be a mainly well-tolerated combination therapy for condyloma acuminatum,which is beneficial to reduce recurrence and improve clinical effectiveness.
