摘 要:目的探讨3.0T MR扩散张量成像(DTI)定量诊断外伤后视神经挫伤的价值。方法对15例单侧眼眶外伤、临床诊断为外伤后单侧视神经挫伤的患者行常规眼眶MRI扫描及视神经DTI扫描。分别测量患侧与健侧眶内段视神经的各向异性分数(FA)值、表观扩散系数(ADC)值。采用配对样本t检验统计分析患侧与健侧视神经FA值与ADC值的差异,P<0.01为有统计学差异。结果常规MRI可以清晰显示患侧与健侧视神经,部分患侧眶内段视神经稍增粗、T2WI信号稍增高,未见明显断裂和移位征象。患侧视神经FA及ADC值分别为0.34±0.07、(1.34±0.19)×10^(-3)mm^(2)/s,健侧视神经FA及ADC值分别为0.57±0.09、(0.95±0.17)×10^(-3)mm^(2)/s,患侧较健侧FA值显著降低(t=8.0,P<0.01)、ADC值显著升高(t=3.2,P<0.01)。结论MR DTI可以定量视神经挫伤改变,有望成为视神经挫伤早期诊断和疗效随访的重要检查手段。Objective To evaluate the value of 3.0T MR diffusion tensor imaging(DTI)in quantitative diagnosis of posttraumatic optic nerve contusion.Methods Routine orbital MRI and DTI of optic nerve were performed in 15 patients with unilateral orbital trauma and clinically diagnosed as unilateral optic nerve contusion after trauma.The fractional anisotropy(FA)value and apparent diffusion coefficient(ADC)value of the optic nerve both in the healthy and affected sides were measured respectively.The values of FA and ADC were statistically analyzed by paired sample t-test,and P<0.01 was statistically significant.Results The routine MRI could clearly show the course and shape of the optic nerve in the healthy side and the affected side.The contused optic nerve in the intraorbital segment was slightly thickened,and the T2 signal of which was slightly increased,while there were no obvious signs of rupture and displacement.The FA and ADC values of the affected optic nerve were 0.34±0.07 and(1.34±0.19)×10^(-3)mm^(2)/s,while the FA and ADC of healthy optic nerve were 0.57±0.09 and(0.95±0.17)×10^(-3)mm^(2)/s.The FA value of the affected side was significantly lower than that of the healthy side(t=8.0,P<0.01),and the ADC value was significantly higher than that of the healthy side(t=3.2,P<0.01).Conclusion MR DTI can quantify the changes of the optic nerve contusion,and it is hopefully to become an important examination means for the early diagnosis and follow-up of therapeutic effect of the patients with optic nerve contusion.
