摘 要:目的探讨Rh血型D、C抗原的分布,并分析其在临床输血中的价值。方法选取平顶山市第二人民医院2020年4月至2021年3月75例拟输血患者,均接受Rh血型鉴定,根据输血制品与患者血型相符情况进行分组,其中39例ABO血型、RhD血型、RhC血型均与输血制品相符合的患者作为观察组,另36例ABO血型、RhD血型与输血制品相符合,RhC血型与输血制品不相符的患者作为对照组,统计Rh血型D、C抗原检测结果及抗体筛查结果,并比较两组输血前后血红蛋白(Hb)、红细胞比容(HCT)水平和不良反应发生率。结果75例患者中Rh血型D抗原阳性41例(54.67%),阴性34例(45.33%),C抗原阳性69例(92.00%),阴性6例(8.00%);输血后两组HCT、Hb水平均高于输血前,且观察组高于对照组(P<0.05);观察组不良反应发生率为5.13%(2/39),低于对照组的25.00%(9/36)(P<0.05)。结论部分需输血患者Rh血型C抗原阳性率较高,输注Rh抗原阳性血液制品并不能提高输血效果,且增加不良反应发生风险,对此类需输血患者应输注无相应抗原的血液制品,临床在进行输血前,应注重筛查Rh血型抗原抗体,以提高输血安全性。Objective To explore the distribution of Rh blood group D,C antigens and analyze its value in clinical blood transfusion.Methods A total of 75 patients undergoing blood transfusion in the Second People’s Hospital of Pingdingshan from April 2020 to March 2021 were selected for Rh blood type identification,and they were divided into two groups according to the consistency of the blood transfusion products and the blood type of the patients.Among them,39 patients with ABO blood type,RhD blood type and RhC blood type consistent with blood transfusion products were conformed to the observation group,and the other 36 patients whose ABO blood type and RhD blood type were consistent with the blood transfusion products,and the RhC blood type was not consistent with blood transfusion products were used as the control group.The Rh blood group D,C antigens detection results and antibody screening results were counted.The levels of hemoglobin(Hb),hematocrit(HCT)and the incidence of adverse reactions were compared between the two groups before and after blood transfusion.Results Among the 75 patients,41 were positive for Rh blood group D antigen(54.67%),and 34 were negative(45.33%),while 69 were positive for C antigen(92.00%),and 6 were negative(8.00%).HCT and Hb levels in the two groups after blood transfusion were higher than those of before blood transfusion,and the observation group were higher than those of the control group(P<0.05).The adverse reaction rate of the observation group was 5.13%(2/39),which was lower than that of the control group 25.00%(9/36)(P<0.05).Conclusion The positive rate of Rh blood group C antigens in some patients requiring blood transfusion is relatively high,and transfusion of Rh antigen-positive blood products does not improve the effect of blood transfusion,and increase the risk of adverse reactions.Such patients should be transfused without corresponding antigens blood products.Before clinical blood transfusion,attention should be paid to screening Rh blood group antigen antibodies to impro
