丙泊酚Wada试验对语言、记忆和运动功能术前评价附15例治疗分析

摘　　要：目的探讨使用丙泊酚Wada试验对语言、记忆力和肢体运动功能的术前评价。方法总结15例大脑半球脑功能区病变患者采用丙泊酚进行Wada试验的临床资料。结果15例患者完成29侧Wada试验,完成语言、记忆和运动功能评估。其中13例患者确定大脑语言优势侧为左侧、2例为右侧;12例患者确定大脑记忆优势为左侧、3例为右侧,其中1例患者大脑语言优势为左侧,记忆优势为右侧。1例左侧额,颞叶脑软化灶并脑发育不良患者对侧侧肌力仅下降致4级,考虑右侧大脑半球对右侧肢体运动区代偿。结论使用丙泊酚进行Wada试验是有效的,选择合适患者,稳定的团队合作,对语言、记忆和运动功能术前评价的准确性,可靠性,安全性起到至关重要作用。Objective To investigate the preoperative evaluation of language,memory,and limbmotor function using propofol Wada test.Methods To summarize the clinical data of Wada test using propofol in 15 patients with cerebral hemispheric functional area lesions.Results 15 patients were assessed for language,memory,and motor function by 29 Wada tests.Language:13 left advantages,2 right advantages.Memory:12 left advantages,3 right advantages.Of these,one had a language advantage on the left and a memory advantage on the right.A patient with a left frontal-temporal cerebral softening foci and dysplasia had only grade 4 contralateral muscle strength,considering the right hemisphere’s compensation for the right limb motor area.Conclusion The Wada test using propofol was valid.Choosing the appropriate patients and stable teamwork play a crucial role in the accuracy,reliability and safety of Preoperative Evaluation of Speech,Memory and Motor Dominance.
