不同分型骨髓增生异常综合征患者骨髓细胞形态学表现对比分析

摘　　要：目的 对比不同分型骨髓增生异常综合征(MDS)患者骨髓细胞形态学表现。方法 回顾性分析2016年1月-2020年6月医院收治86例MDS患者临床资料,所有患者病例资料均完整,参照WHO标准评估所有MDS患者的分型,比较不同WHO分型MDS患者的骨髓细胞形态学指标,包括骨髓增生程度、三系造血细胞检出率。结果 86例MDS患者中WHO分型占比居前三的分别是MDS-U型、RAEB-2型、RAEB-1型,占比分别为26. 74%、23. 26%、12. 79%;不同WHO分型MDS患者的骨髓增生程度比较,差异无统计学意义(P> 0. 05);RAEB-2型的病态粒系检出率高于其他分型,MDS-U型患者的病态红系检出率高于其他分型,MDS-5q-型患者的病态巨核系检出率高于其他分型,差异有统计学意义(P<0. 05)。结论 不同分型MDS患者的骨髓细胞形态学存在差异,可考虑根据骨髓细胞形态学诊断MDS,并根据不同分型采取不同治疗方案。Objective To compare the morphological features of bone marrow cells in patients with different types of myelodysplastic syndrome(MDS).Methods The clinical data of 86 patients with MDS treated to the hospital from January 2016 to June 2020 were retrospectively analyzed,the medical data of all patients were complete,the classification of all patients with MDS were evaluated according to WHO criteria,the morphological indexes of bone marrow cells in MDS patients with different WHO classification were compared,including the degree of myelodysplasia and the detection rate of three line hematopoietic cells.Results Among the 86 patients with MDS,the top three of the proportion of WHO classification were MDS-U type,RAEB-2 type and RAEB-1 type,accounting for 26.74%,23.26%and 12.79%,respectively;there was no statistical significance on the difference of the degree of myelodysplasia in MDS patients with different WHO classification(P>0.05);the detection rate of morbid granulocyte in RAEB-2 types was higher than that of other subtypes,the detection rate of morbid erythroid in MDS-U type patients was higher than that in other subtypes,and the detection rate of morbid megakaryotes in MDS-5q-type patients was higher than that in other subtypes,with the differences statistical significant(P<0.05).Conclusion There are differences in morphology of bone marrow cells in patients with different types of MDS,MDS can be considered to be diagnosed according to the morphology of bone marrow cells,and different treatment schemes can be adopted according to different types.
