摘 要:目的探讨帕金森病患者语音的元音声学特征,并筛选出敏感的元音声学评估指标。方法2019年10月至2020年1月于上海市第一人民医院共收集26例1~3期帕金森病患者(帕金森病组,男、女各13例,年龄57~81岁,平均70.69±5.62岁),并同期收集22例年龄相似的健康成人(对照组,男12例,女10例),分别朗读短文,从短文的部分音节/da/、/ta/、/ba/、/yi/、/di/、/ni/、/shu/、/bu/、/fu/中提取单元音/a/、/i/、/u/进行声学分析,并计算出下颌距、舌距、元音空间面积(vowel space area,VSA)、元音清晰度指数(vowel articulation index,VAI)、共振峰集中率(formant centralization ratio,FCR),比较两组结果。结果两组中男性下颌距、舌距、VSA、VAI极显著小于女性,FCR极显著大于女性(分别为F=34.158,P<0.001;F=50.584,P<0.001;F=96.317,P<0.001;F=13.390,P=0.001;F=12.724,P=0.001);帕金森病组VAI极显著小于正常对照组,FCR极显著大于正常对照组(分别为F=9.209,P=0.004;F=8.690,P=0.005)。结论1~3期帕金森病患者的元音发音较正常人趋于集中化,且清晰度下降,VAI和FCR作为评估帕金森病患者语音的元音特征的有效声学指标,可区分帕金森病患者与正常人。Objective This study was aimed to explore the vowel acoustic characteristics in Parkinson’s disease and to screen the sensitive indicators for evaluation.Methods A total of 26 patients with Parkinson’s disease(13 males and 13 females aged from 57~81 years old with average of 70.69±5.62 years old)and 22 healthy adults(12 males and 10 females)were included in this stues in Shanghai General Hospital.The All participants were required to read a short essay to extract vowels/a,i,u/from 9 monosyllables(da,ta,ba,yi,di,ni,shu,bu,fu),then analyzed and calculated jaw distance,tongue distance,vowel space area(VSA),vowel articulation index(VAI),and formant centralization ratio(FCR).Results Jaw distance,tongue distance,VSA and VAI of males were significantly smaller while FCR was significantly larger than those of females(F=34.158,P<0.001;F=50.584,P<0.001;F=96.317,P<0.001;F=13.390,P=0.001;F=12.724,P=0.001).VAI and FCR in Parkinson’s disease were significantly different from healthy controls that VAI was smaller and FCR was larger(F=9.209,P=0.004;F=8.690,P=0.005).Conclusion Vowel production in Parkinson’s disease was more centralized than healthy adults,resulted in the reduction of speech intelligence.VAI and FCR can be used as effective acoustic indicators to evaluate vowel production sensitively in Parkinson’s disease.
