分泌性中耳炎患耳的宽频声导抗特征研究

摘　　要：目的对比分泌性中耳炎患者与正常成年人的宽频声导抗声能吸收率的差异,探讨宽频声导抗测试对分泌性中耳炎的诊断价值。方法选取126例(252耳)正常成年人为对照组(正常组),64例(89耳)分泌性中耳炎患者为实验组(分泌性中耳炎组),对两组分别在外耳道压力为0 daPa和峰压时进行宽频声导抗测试,比较两组声能吸收率特征。结果无论是外耳道压力为峰压还是0 daPa时,分泌性中耳炎组500 Hz以上声能吸收率均低于正常组,差异有统计学意义(P<0.05),尤其600~2000 Hz最为明显。外耳道压力为峰压时,正常组声能吸收率的两个峰值点对应的频率为1122 Hz和4238 Hz,分泌性中耳炎组为944 Hz和3668 Hz,峰值点均前移;外耳道压力为0 daPa时,正常组声能吸收率的两个峰值点对应的频率为1122 Hz和4238 Hz,分泌性中耳炎组为648 Hz和3776 Hz,峰值点亦前移。正常组的中耳共振频率为912±243.01 Hz,分泌性中耳炎组为396±76.45 Hz,分泌性中耳炎组的中耳共振频率明显减小,差异有统计学意义(P<0.05)。结论分泌性中耳炎患耳在600~2000 Hz宽频声能吸收率下降最明显,且中耳共振频率明显小于正常人。Objective To compare the wideband absorbance between secretory otitis media and normal adults,and to explore the diagnostic value.Methods A total of 126 normal adults(252 ears)were selected as the control group,and 64 patients(89 ears)with secretory otitis media were selected as the experimental group.The two groups were subjected to wideband acoustic immittance tests to compare the characteristics of the wideband absorbance when in peak pressure and 0 daPa respectively.Results No matter whether the peak pressure or 0 daPa,the wideband absorbance above 500 Hz in the secretory otitis media group was lower than that of the normal group,and the difference between the two was statistically significant(P<0.05),especially 600~2000 Hz was the most obvious.Under the peak pressure,the two peak points of the absorbance of the normal group at 1122 Hz and 4238 Hz,and the secretory otitis media group were at 944 Hz and 3668 Hz,indicating that the peak points moved forward.Under 0 daPa,the two peak points of the absorbance of the normal group were at 1122 Hz and 4238 Hz,and the secretory otitis media group were at 648 Hz and 3776 Hz,showing that the peak points also moved forward.The resonance frequency of the normal group was 912±243.01 Hz,while the other group was 396±76.45 Hz,with a significant reduction of the secretory otitis media group(P<0.05).Conclusion Secretory otitis media has the most significant decrease in the wideband absorbance of 600~2000 Hz,and the resonance frequency of the middle ear is significantly lower than that of the normal people.
