摘 要:目的研究紫癜性肾炎(HSPN)患儿血清B淋巴细胞活化因子(BAFF)、中期因子(MK)水平与肾损伤及Th17和调节性T细胞平衡的关系。方法将复旦大学附属中山医院青浦分院儿科2017年1月至2020年1月收治的71例HSPN患儿作为研究组,另取同期医院收治的过敏性紫癜(HSP)患儿70例作为HSP组,同期于医院体检的健康志愿儿童70例作为对照组。检测并比较三组受试者血清BAFF、MK水平、肾损伤指标水平、Th17和调节性T细胞比例以及Th17细胞相关因子水平,采用Pearson相关性分析血清BAFF、MK水平与肾损伤及Th17和调节性T细胞平衡的相关性。结果研究组患儿的血清BAFF水平明显低于HSP组和对照组,而血清MK水平明显高于HSP组和对照组,其中HSP组患儿的血清MK水平明显高于对照组,差异均有统计学意义(P<0.05),HSP组患儿的血清BAFF水平与对照组比较无统计学差异(P>0.05);研究组患儿的尿微量白蛋白、尿转铁蛋白及尿β微球蛋白水平明显高于HSP组和对照组,差异均有统计学意义(P<0.05),而HSP组患儿的尿微量白蛋白、尿转铁蛋白及尿β微球蛋白水平分别与对照组比较,差异均无统计学意义(P>0.05);三组受试者间的尿素氮及肌酐水平比较差异均无统计学意义(P>0.05);研究组患儿的Th17细胞比例及血清白细胞介素-6(IL-6)、白细胞介素-17(IL-17)水平明显高于HSP组,而调节性T细胞比例明显低于HSP组,且HSP组患儿的Th17细胞比例以及血清IL-6、IL-17水平明显高于对照组,而调节性T细胞比例明显低于对照组,差异均有统计学意义(P<0.05);经Pearson相关性分析结果显示,血清BAFF与Th17细胞比例、尿微量白蛋白、尿转铁蛋白、尿β微球蛋白及血清IL-6、IL-17水平均呈负相关(P<0.05),与调节性T细胞比例呈正相关(P<0.05);血清MK与Th17细胞比例、尿微量白蛋白、尿转铁蛋白、尿β微球蛋白及血清IL-6、IL-17水平均呈正相关(P<0.05),与调节性T细胞比例呈Objective To investigate the relationship between serum B lymphocyte activating factor(BAFF)midkine(MK)levels and renal injury,Th17 and regulatory T cell balance in children with Henoch-Sch?nlein purpura nephritis(HSPN).Methods Seventy-one children with HSPN admitted to the Department of Pediatrics at Zhongshan Hospital Affiliated to Fudan University(Qingpu Branch)from January 2017 to January 2020 were selected as the study group,70 children with Henoch-Sch?nlein purpura(HSP)admitted to the hospital during the same period were selected as the HSP group,and 70 healthy volunteer children who underwent physical examination in the hospital during the same period were selected as the control group.The serum levels of BAFF and MK,and renal injury indicators were detected and compared among the three groups,as well as the proportion of Th17 and regulatory T cells,and the levels of Th17 cell-related factors.Pearson correlation was used to analyze the correlation between serum BAFF or MK levels and renal injury indicators,the balance of Th17 and regulatory T cells.Results The serum BAFF level of the study group was significantly lower than that of the HSP group and the control group,while the serum MK level of the study group was significantly higher than that of the HSP group and the control group;among them,the serum MK level of the HSP group was significantly higher than that of the control group;all differences were statistically significant(P<0.05);while there was no statistically significant difference in serum BAFF level between the HSP group and the control group(P>0.05).The levels of urinary microalbumin,urinary transferrin,and urinaryβ-microglobulin in the study group were significantly higher than those in the HSP group and the control group(P<0.05),while there was no statistically significant difference in the levels of urinary microalbumin,urinary transferrin,and urinaryβ-microglobulin between the HSP group and the control group(P>0.05).There was no significant difference in the levels of urea nitrogen and
