摘　　要：目的通过对某三甲医院住院病案首页数据的统计,分析影响老年住院择期手术患者并发症的因素,为医政管理提供线索。方法抽取某院2017年1月1日-2019年12月31日的老年住院择期手术患者信息,进行手术并发症描述与推断分析,采用例数、比率等指标,使用卡方检验、秩和检验等方法进行描述。结果男性、高龄、四级手术并发症发生率高,P<0.05,男性0.86%大于女性0.62%,70岁以上0.84%大于60岁~69岁0.68%,四级1.42%大于非四级0.40%;所调查32301例择期手术中,感染178例、出血或血肿23例等例数较多。结论性别、年龄、手术级别对部分手术并发症发生率有影响。对男性、高龄、四级手术患者要加强医疗质量与安全管理,利用好信息化平台,不断完善预警监测体系,探索多学科协作模式,加强手术分级授权管理,严格遵循医疗质量管理制度。Objective Through the statistics of the data on the first page of hospitalized medical records in a third-class hospital,the factors affecting the complications of elderly hospitalized patients undergoing elective surgery were analyzed in order to provide clues for medical management.Methods The information of elderly patients undergoing elective surgery in a hospital from January 1,2017 to December 31,2019 was extracted,and the operative complications were described and analyzed.The number of cases,ratio and other indicators were used,and chi-square test and rank sum test were used to describe them.Results The incidence of complications in male,advanced age and grade 4 operation was high(P<0.05).The incidence of complication in male(0.86%)was higher than that in female(0.62%).Over 70 years old(0.84%)was greater than(0.68%)in 60-69 years old,and 1.42% in grade 4 was greater than 0.40% in non-grade 4.Among the 32301 cases investigated,there were more cases of infection(178 cases),hemorrhage or hematoma(23 cases).Conclusion Sex,age and surgical grade have influence on the incidence of some operative complications.For male,elderly and grade 4 surgical patients,it is necessary to strengthen the management of medical quality and safety,make good use of the information platform,constantly improve the early warning and monitoring system,explore the mode of multi-disciplinary cooperation,and strengthen the graded authorization management of surgery,strictly follow the medical quality management system.
