摘  要:目的探讨慢性乙型肝炎病毒(HBV)宫内感染孕妇血清长链非编码RNA GAS5(LncRNA GAS5)、锌指蛋白1(ZEB1)的表达及其临床意义。方法收集2017年6月1日-2018年10月30日HBV感染孕妇90例作为研究对象,根据新生儿脐血检测结果将其分为HBV宫内感染组40例与未感染组50例,选取同期分娩的非HBV感染的孕妇55例作为对照组。采用实时荧光定量聚合酶链反应(qRT-PCR)检测血清GAS5、ZEB1 mRNA的表达水平;采用酶联免疫吸附试验(ELISA)检测血清白细胞介素10(IL-10)、白细胞介素17(IL-17)、白细胞介素21(IL-21)水平;采用ELISA法检测血清白细胞介素12(IL-12)、白细胞介素18(IL-18)水平;应用流式细胞仪检测Toll样受体4(TLR4)水平;采用Pearson法分析HBV宫内感染孕妇GAS5、ZEB1与IL-10、IL-17、IL-21、IL-12、IL-18、TLR4的相关性;采用Logistic多元回归分析影响HBV宫内感染的相关因素。结果与对照组相比,未感染组与宫内感染组孕妇血清GAS5的表达水平显著降低,ZEB1 mRNA的表达水平显著升高,IL-10、IL-17、IL-21水平显著升高,IL-12、IL-18、TLR4水平显著降低,未感染组与宫内感染组相比差异有统计学意义;GAS5与ZEB、IL-10、IL-17、IL-21呈负相关,ZEB1与IL-10、IL-17、IL-21呈正相关;GAS5与IL-12、IL-18、TLR4呈正相关,ZEB1与IL-12、IL-18、TLR4呈负相关;Logistic多因素分析显示GAS5、ZEB1是影响HBV宫内感染的独立危险因素。结论GAS5的表达水平降低与ZEB1的表达水平升高可能是HBV宫内感染的危险因素。Objectives To investigate the expression of LncRNA GAS5 and ZEB1 in serum of pregnant women with HBV intrauterine infection and its clinical significance.Methods To collect 90 cases of pregnant women with HBV infection from June 1st,2017 to October 30th,2018 as the research objects,and they were divided into 40 cases of HBV intrauterine infection group and 50 cases of uninfected group based on the results of umbilical cord blood test.55 cases of non-HBV infected pregnant women who delivered at the same time were selected as control group.The expression levels of GAS5 and ZEB1 mRNA in serum were detected by qRT-PCR.Serum IL-10,IL-17,and IL-21 levels were detected by ELISA.Serum IL-12 and IL-18 levels were detected by ELISA.Flow cytometry was used to detect the level of TLR4.The correlation between GAS5,ZEB1 and IL-10,IL-17,IL-21,IL-12,IL-18,TLR4 in pregnant women with HBV intrauterine infection was analyzed by Pearson method.Logistic multiple regression was used to analyze the related factors affecting HBV intrauterine infection.Results Compared with the control group,the expression levels of GAS5 in the serum of pregnant women in the uninfected group and the intrauterine infection group were significantly reduced,and the expression levels of ZEB1 mRNA were significantly increased.The levels of IL-21 were significantly increased,and the levels of IL-12,IL-18,and TLR4 were significantly decreased.The difference between the uninfected group and the intrauterine infection group was statistically significant.GAS5 was negatively correlated with ZEB,IL-10,IL-17,and IL-21,and ZEB1 was positively correlated with IL-10,IL-17,and IL-21.GAS5 was positively correlated with IL-12,IL-18,and TLR4,and ZEB1 was negatively correlated with IL-12,IL-18,and TLR4.Logistic multivariate analysis showed that GAS5 and ZEB1 were independent risk factors affecting HBV intrauterine infection.Conclusions Decreased expression of GAS5 and increased expression of ZEB1 might be risk factors for HBV intrauterine infection.
