中药熏蒸联合玻璃酸钠滴眼液治疗干眼症44例临床观察

摘　　要：目的:观察中药熏蒸联合玻璃酸钠滴眼液治疗干眼症的临床疗效。方法:将90例干眼症患者随机分为治疗组(44例)与对照组(46例)。对照组采用玻璃酸钠滴眼液滴眼治疗,治疗组在对照组治疗的基础上加用清肝明目中药局部熏蒸治疗,疗程均为1个月。比较2组治疗前后国际常用经典眼表疾病指数(OSDI)评分及泪膜破裂时间(BUT)、基础泪液分泌试验(SIT)值的变化情况,根据临床症状及SIT值的结果评定2组综合疗效。结果:总有效率治疗组为90.91%(40/44),对照组为76.09%(35/46),2组比较,差异有统计学意义(P<0.05)。2组OSDI评分、BUT、SIT值治疗前后组内比较及治疗后组间比较,差异均有统计学意义(P<0.05或P<0.01)。结论:中药熏蒸联合玻璃酸钠滴眼液能有效改善干眼症眼表刺激症状,延长泪膜破裂时间,改善泪液分泌功能,疗效显著。Objective: To investigate the clinical effect of traditional Chinese medicine(TCM) fumigation combined with sodium hyaluronate eye drops in the treatment of xerophthalmia.Methods: A total of 90 patients with xerophthalmia were randomly divided into treatment group with 44 patients and control group with 46 patients.The patients in the control group were given sodium hyaluronate eye drops alone, and those in the treatment group were given local TCM fumigation and steaming therapy with clearing liver-fire and eyesight-improving medicine in addition to the treatment in the control group.The course of treatment was 1 month for both groups.The two groups were compared in terms of the changes in Ocular Surface Disease Index(OSDI),tear break-up time(TBUT),and Schirmer I test(SIT),and overall response was evaluated based on clinical symptoms and SIT results.Results: There was a significant difference in overall response rate between the treatment group and the control group [90.91%(40/44) vs 76.09%(35/46),P<0.05].Both groups had significant changes in OSDI score, TBUT,and SIT results after treatment, and there were significant differences in these indices between the two groups after treatment(P<0.05 or P<0.01).Conclusion: TCM fumigation combined with sodium hyaluronate eye drops has a marked clinical effect in the treatment of xerophthalmia and can effectively improve the symptom of ocular surface stimulation, delay TBUT,and improve the function of tear secretion.
