张迎春活血化瘀法安胎理论及临床应用

摘　　要：先兆流产为妇科常见病,属于中医学“胎漏”“胎动不安”“滑胎”等范畴,其辨证分型主要有肾虚、血热、气血虚弱及血瘀等证,其中血瘀证是导致现代女性流产不容忽视的重要因素,张迎春教授为第六批全国老中医药学术继承工作指导老师,认为孕后瘀血的成因包括阴虚内热血瘀、气滞血瘀及寒凝血瘀等,同时结合现代检测技术,对孕后B超子宫动脉RI值进行监测。并在辨证论治的基础上治疗“血瘀证”胎漏、胎动不安及滑胎,审证求因,给予以清热凉血、滋阴养血及暖宫散寒等安胎的同时,灵活变通,适当应用活血化瘀药如当归、川芎、丹参、三七、桃仁等,使得瘀祛脉络通,胎有所养则胎自安,从而提高临床治愈率。Threatened abortion is a common gynecologic disease,and the traditional Chinese medicine belongs to the category of"fetal leakage","restlessness of the fetus"and"slippery fetus".Its syndrome differentiation mainly includes kidney deficiency,blood heat,weakness of Qi and blood,and blood stasis,Professor ZHANG Ying-chun is the teacher of the sixth batch of academic inheritance of traditional Chinese medicine,she summarized her clinical experience for many years,consider that the causes of blood stasis after pregnancy include Yin deficiency,heat and blood stasis,Qi stagnation and blood stasis due to cold coagulation and blood stasis,etc.,the RI value of uterine artery was monitored by B-mode ultrasound after pregnancy.Professor ZHANG Yingchun,on the basis of syndrome differentiation and treatment,treated the"blood stasis syndrome"of fetal leakage,restless fetal movement and systosis,examined the cause of syndrome,gave the measures of clearing away heat and cooling blood,nourishing Yin and blood,warming the uterus and dispersing cold,and so on,while keeping the fetus stable,she is flexible,proper application of blood-activating and stasis-removing drugs such as Angelica,Chuanxiong,Danshen,Panax Notoginseng,peach kernel,etc.
