摘 要:高精度直流电刺激(High-definition Transcranial Direct Current Stimulation,HD-tDCS)可以通过靶向特定的大脑区域来调节神经活动,有益于意识障碍(Disorder of consciousness,DOC)患者的康复。追踪记录HD-tDCS治疗的27例DOC患者在4个治疗阶段的昏迷恢复量表(Coma Recovery Scale-Revised,CRS-R)评分和脑电图,探究HD-tDCS对DOC患者脑电特征变化的影响。研究结果发现,经过HD-tDCS治疗后,CRS-R评分显著提高;刺激区和非刺激区脑电特征无显著差异;脑电功率谱低频段能量下降,高频段能量上升,脑电非线性特征值提高,脑功能复杂性增强,从神经电生理学角度说明了HD-tDCS能够帮助DOC患者恢复意识水平。In recent years,transcranial direct current stimulation(tDCS)has been shown to be beneficial to the rehabilitation of patients with disorder of consciousness(DOC).At present,HD-tDCS has proved to be helpful for the treatment of DOC patients,but the regulation mechanism needs further research.Therefore,to explore the effect of HD-tDCS on the treatment of DOC patients,the research tracked 27 DOC patients treated with HD-tDCS,recorded the EEG and Coma Recovery Scale scores before treatment,after a single treatment,7 days and 14 days after treatment,and analyzed the changes in EEG characteristics.The results showed that:after HD-tDCS treatment,the CRS-R score increased significantly;there is no significant difference in EEG characteristics between stimulated and non-stimulated areas;the low-frequency energy of the EEG power spectrum decreases,the high-frequency energy increases,the nonlinear characteristic value of EEG increases,and the complexity of brain functions increases.These changes show that HD-tDCS can help patients with DOC restore their level of consciousness,from the perspective of neuroelectrophysiology.
