某儿科医院儿童药品审方规则库建立的体会与思考

摘　　要：目的:通过解决南京医科大学附属儿童医院儿童药品规则库建立过程中存在的问题,提高合理用药软件在儿科医院的适应性,促进儿童合理用药水平的提升。方法:从"儿童药品剂量规则设置"、"儿童超说明书用药规则设置"、"儿童个性化给药规则设置"、"医院药品管控规则设置"等4个方面阐述建立适应性更高的儿童药品规则库角度和思路。结果:初步建立该院儿童药品规则库,自2019年至2020年住院医嘱合理率不断提高,不合理医嘱类别占比逐渐下降,实现了从信息化层面保障儿童合理用药。结论:儿童药品规则库的建立既保障了儿童患者用药安全,又体现了药师的的专业技术水平和药事服务能力,对于其中存在的问题也需要进一步完善。OBJECTIVE To resolve the existing problems in the establishment process of children's drug rule base at Cildren's Hospital of Nanjing Medical University,enhance the adaptability of rational drug use software in pediatric hospitals and promote rational drug use level in children.METHODS Perspectives and concepts of establishing children's drug rule base with a higher adaptability were explored from four aspects of"setting of children's drug dose rule","setting of children's off-label drug use rule","setting of children's personalized drug administration rule"and"setting of hospital drug control rule".RESULTS The rule base of children's drugs at a pediatric hospital has been preliminarily established from 2019 to 2020,rational rate of in-patient medical orders has continuously spiked and the proportion of unreasonable medical orders has been gradually declined.Thus the protection of children's rational drug use was attained from the information level.CONCLUSION The establishment of children's drug rule base not only guarantees the drug safety of children,but also reflects the professional technical level and pharmaceutical service capability of pharmacists.The existing problems should be further improved.
