桃核承气汤在防治下肢深静脉血栓形成中的临床疗效研究

摘　　要：目的评价桃核承气汤联合规范西医方案治疗下肢深静脉血栓形成的近远期疗效。方法选取收治的下肢深静脉血栓形成急性期患者87例,根据治疗方法不同分为治疗组(43例)和对照治疗(44例),对照组治疗方法为西医规范方案,治疗组在此基础上联合桃核承气汤,比较两组治疗前后D-二聚体、症状评分、生活质量评分、血栓复发率、出血风险、血栓形成后综合征的发生率等指标。结果治疗组较对照组D-二聚体水平恢复正常速度快(P<0.05);早期可以显著减轻患者症状,提高生活质量(P<0.05);血栓复发率和出血事件无显著差异(P>0.05);治疗组较对照组可以降低血栓形成后综合征的发生率、降低Villalta评分(P<0.05)。结论桃核承气汤联合规范西医方案治疗急性下肢DVT能够早期有效缓解患者症状,改善生活质量,同时能够降低PTS发生率,提升近远期总体治疗效果。Objective To evaluate the short-term and long-term efficacy of Taohe Chengqi decoction combined with standardized western medicine in the treatment of lower extremity deep vein thrombosis.Methods 87 patients with acute deep venous thrombosis of lower extremities were selected and divided into treatment group(43 cases)and control group(44 cases)according to different treatment methods.The treatment method of control group was western medicine standard method,and the treatment group was combined with Taohe Chengqi decoction on this method.D-dimer,VEINES-Sym,VEINES-QOL,and thrombus recurrence of the two groups before and after treatment,bleeding risk,incidence of post thrombotic syndrome were compared.Results The D-dimer level in the treatment group returned to normal faster than that in the control group(P<0.05).In the early stage,compared with the control group,the symptoms of patients were significantly reduced and the quality of life was improved(P<0.05).The recurrence rate of thrombosis and bleeding events had no significant difference(P>0.05).The incidence of post thrombotic syndrome and Villalta score in the treatment group were lower than those in the control group(P<0.05).Conclusion Taohe Chengqi decoction combined with standardized western medicine in the treatment of acute lower extremity deep vein thrombosi can effectively relieve the symptoms of patients,improve the quality of life,reduce the incidence of post thrombotic syndrome,and improve the overall treatment effect.
