胰岛移植缓解糖尿病大鼠肾脏纤维化的实验研究

摘  要:目的观察海藻酸钠-聚赖氨酸-海藻酸钠(APA)微囊化胰岛细胞移植对糖尿病肾病大鼠早期肾脏病变的疗效,并探讨其可能机制。方法SD大鼠单剂链脲佐菌素腹腔注射建立糖尿病模型实验分3组,糖尿病肾病组:糖尿病大鼠建模8周后,随机选择加入该组;微囊移植组:糖尿病大鼠建模后8周,采用APA微囊化胰岛细胞腹腔移植;正常对照组:移植后4周取各组大鼠24 h尿,检测尿蛋白总量;并且随机检测各组血糖收集各组肾组织标本进行电镜检查和免疫组化检测TGF-β1和CTGF在肾脏中的表达;检测肾动脉平滑肌中MCP-1的表达。结果APA微囊化胰岛移植可以降低糖尿病大鼠血糖和减少24 h尿蛋白量(P<0.05)肾脏电镜结果显示,微囊移植组大鼠肾小球滤过屏障结构明显改善;免疫组化结果提示,相比糖尿病肾病组,微囊移植组大鼠肾脏中TGF-β1和CTGF蛋白表达减少(P<0.05),肾动脉中MCP-1的表达明显降低(P<0.05),结论APA微囊化胰岛移植可以改善大鼠早期糖尿病肾病进展。Objective To observe the effect of Alginic polylysine alginic(APA)microencapsulated islet transplantation on early renal lesions of diabetic nephropathy rats and explore its possible mechanism.Methods Diabetic model was established by intraperitoneal injection of streptozotocin.The experiment was divided into three groups.Diabetic nephrop«ithy group:diabetic rats were randomly selected to join the group 8 weeks after modeling;microencapsulated transplantation group:diabetic rats 8 weeks after modeling,using APA microencapsulated islet abdominal transplantation;normal control group:4 weeks after transplantation,urine of rats in each group was collected for 24 hours to detect the total amount of urine protein,and blood glucose of each group was randomly detected.Kidneys were detected by electron microscopy.The expression of TGF-β1 and CTGF in kidney were tested by immunohistocheniistry.The expression of MCP-1 in renal artery smooth muscle was tested by immunohistochemistry.Results Microencapsulated islet transplantation can reduce blood glucose and 24 h urine protein in rats(P<0.05).The results of renal electron microscopy suggested that The structure filtration barrier in the microencapsulated transplantation group significantly improved.Compared with the diabetic nephropathy group,the expression of TGF-β1 and CTGF in the niicrocapsule transplantation group decreased significantly(P<0.05).Compared with diabetic nephropathy group,the expression of MCP-1 in renal artery of microcapsule transplantation group was significantly lower(P<0.05).Conclusion APA microencapsulated islet transplantation can improve the development of early diabetic nephropathy.
