双钢板联合内侧植骨治疗肱骨近端粉碎骨折疗效观察

摘　　要：目的探讨内外侧双接骨板结合内侧植骨治疗肱骨近端粉碎性骨折的疗效方法2018年8月至2020年8月采用内外侧双接骨板结合内侧植骨支撐治疗肱骨近端骨折12例,男8例,女4例;年龄25〜69岁,平均50.8岁受伤原因:跌倒外伤3例,高处坠落伤3例,交通事故6例观察手术并发症情况、骨折愈合情况并根据相关肩关节功能评分标准评价疗效结果手术时间90〜150 min,平均115 mm,术中出血量100〜400 mL,平均280 mL,手术中无腋神经损伤,所有患者均予以随访,随访时间6〜15个月,平均13个月,所有患者骨折均愈合,无肱骨头内翻畸形,骨不连,内固定失效,螺钉穿出肱骨头等并发症发生,恨据肩关节功能评分,优6例,良4例,一般2例结论采用内外侧双接骨板结合内侧植骨支撑治疗肱骨近端骨折手术安全,疗效可靠,并发症发生率低。Objective To investigate the clinical effect of medial and lateral double bone plates combined with medial bone grafting in the treatment of comminuted fractures of proximal humenas.Methods From August 2018 to August 2020,12 cases(8 males and 4 females)of proximal huniems fractures were treated by medial and lateral double bone plates combined with medi.il hone graft support,Age 25〜69 years old,average 50.8 years old.Causes of injury:3 cases of fall injury,3 cases of high fall injury,6 cases of traffic accidents.The surgical complications and fracture healing were observed,and the efficacy was evaluated according to the relevant shoulder joint function scoring criteria.Results The operation time was 90〜150 min,with an average of 115 min.Tht、intraoperative hlood loss was 100〜400 mL,with an average of 280 mL,There was no axillary nerve injury during the operation.All patients were followed up from June to 15 months,with an average of 13 months.Fracture healing was observed in all patients,without varus head deformity,bone nonunion,failure of internal fixation,and complications of screw perforation of humeral head and head.According to the shoulder joint flinction score,6 cases were excellent,4 cases were good,and 2 cases were average.Conclusion The treatment of proximal humenis fracture with medial and lateral double bone plates combined with medial bone grafting is safe,reliable and has low complication rate.
