2型糖尿病早期肾脏损害患者血清神经调节蛋白4、鸢尾素水平变化及临床意义

摘　　要：目的探讨T2DM早期肾脏损害(EDN)患者血清神经调节蛋白4(Nrg4)、鸢尾素(Irisin)水平变化及临床意义。方法选取2019年8月至2020年11月于我院内分泌科住院治疗的单纯T2DM患者46例(T2DM组)、EDN患者45例(EDN组);同期于我院体检健康者46名为正常对照(NC)组。测定各组血清Nrg4、Irisin水平。结果 NC、T2DM、EDN组血清Nrg4[(20.03±3.52)vs(14.42±2.76)vs(7.32±2.84)pg/ml,P<0.05]及Irisin[(275.69±70.32)vs(202.37±71.19)vs(158.26±43.87)ng/ml,P<0.05]均依次降低。Pearson相关分析显示,血清Nrg4水平与Irisin、eGFR、葡萄糖目标范围内时间(TIR)呈正相关(P<0.01),与FPG、TG、FIns、HOMA-IR、HbA_(1)c、UACR、胱抑素C、β2-微球蛋白呈负相关(P<0.05)。Logistic回归分析显示,TIR、HbA_(1)c、Nrg4、Irisin是EDN的影响因素。结论 EDN患者血清Nrg4、Irisin水平降低。Objective To explore the changes and clinical significance of serum Nrg4 and Irisin in patients with type 2 diabetic early kidney damage(EDN).Methods From August 2019 to November2020,46 patients with simple T2 DM(T2 DM group),45 patients with early type 2 diabetic nephropathy(EDN group)and 46 healthy people with normal glucose tolerance(NC group)were selected. Serum levels of nrg4 and irisin were measured.Results Both serum Nrg4 levels[(20. 03±3. 52)vs(14. 42±2. 76)vs(7. 32±2. 84)pg/ml,P<0. 05]and Irisin[(275. 69±70. 32)vs( 202. 37±71. 19)vs(158. 26±43. 87)ng/ml,P<0. 05]were decreased gradually from NC group,T2 DM group to EDN group. Pearson correlation analysis showed that serum Nrg4 levels were positively correlated with Irisin,e GFR,and time in target range(TIR)(P<0. 01),and negatively correlated with FPG,TG,FIns,HOMA-IR,Hb A1 c,UACR,Cystatin C and β2-microglobulin(P<0. 05). Logistic regression analysis showed that TIR,Hb A_(1) c,Nrg4,Irisin were the influencing factors of EDN.Conclusion The serum levels of Nrg4 and Irisin decreased in EDN patients,which was closely related to EDN.
