摘  要:目的:探讨脑电图分级对神经外科昏迷患者的评估价值。方法:回顾性分析2013年1月至2020年12月收治的60例不同病因昏迷患者,将视频脑电图监测结果进行Synek分级和格拉斯哥昏迷评分(GCS)。对脑电图分级和GCS评分进行相关分析。结果:脑电图背景活动以慢波为主(θ波33例、δ波或纺锤波昏迷13例)。脑电图分级与GCS评分呈负相关(r=-0.508,P<0.05)。临床预后良好48例、不良12例。脑电图良好组的临床预后良好率明显高于脑电图不良组(97.3%比52.2%,P<0.05)。结论:Synek脑电图分级能反映神经外科昏迷患者的脑功能损伤程度,其分级越高,GCS评分越低,预后越差。Objective: To explore the evaluate value of electroencephalography(EEG) grading in neurosurgical coma patients. Methods: The video EEG monitoring results of 60 coma patients with different causes treated from January 2013 to December 2020 were analyzed retrospectively. Synek grading and Glasgow coma score(GCS) were performed. The correlation between EEG grading and GCS score was analyzed. Results: The EEG background activity was mainly slow wave(33 cases θ wave, 13 cases δ wave or spindle wave coma). There was a negative correlation between EEG grading and GCS score(r=-0.508, P<0.05). Clinical recovery was good in 48 cases and poor in 12 cases. The good rate of clinical prognosis in the group with good EEG was significantly higher than that in the group with bad EEG(97.3% vs. 52.2%, P<0.05). Conclusion: Synek EEG grading can reflect the degree of brain function injury in neurosurgical coma patients. The higher the grading, the lower the GCS score, the worse the prognosis.
