摘　　要：目的:研究EP受体在慢性鼻-鼻窦炎伴鼻息肉(chronic rhinosinusitis with nasal polyps,CRSw NP)中的表达及意义。方法:收集20例嗜酸粒细胞性CRSw NP (eosinophilic CRSw NP,ECRSw NP)、20例非嗜酸粒细胞性CRSw NP(noneosinophilic CRSw NP,non-ECRSwNP)患者息肉和14例正常对照组鼻腔钩突黏膜。免疫组织化学和Western blot技术检测各组鼻组织中四种EP受体亚型蛋白的表达;对连续切片行免疫组化染色,检测EP受体与活化的嗜酸粒细胞之间的关系;用Real-time PCR检测各组EP受体和IL-5/IL-13 m RNA的表达水平。结果:EP受体主要表达于鼻黏膜上皮、腺体和上皮下炎症细胞,EP1受体选择性表达于上皮下炎症细胞。与对照组和non-ECRSwNP相比较,ECRSw NP组中EP1 m RNA和蛋白表达均上调,而三组间EP2、EP3和EP4受体的表达无明显差异。连续切片免疫组化染色示,EP1阳性的嗜酸粒细胞占EP1阳性总细胞数的50%。息肉组织EP1 m RNA与IL-5(r=0.55;P <0.001)、IL-13(r=0.69;P<0.001)m RNA的表达水平呈正相关。结论:ECRSw NP中EP1的表达上调与大量的嗜酸粒细胞等浸润有关。EP1受体可能通过趋化和活化嗜酸粒细胞参与ECRSw NP组织炎症的发生和发展。Objective:To investigate clinical significance and expression of the four EP receptor subtypes,EP1,EP2,EP3 and EP4 receptors in chronic rhinosinusitis with nasal polyps(CRSw NP)patients.Methods:Nasal biopsies were obtained from 14 controls,20 eosinophilic chronic rhinosinusitis with nasal polyps(CRSw NP)and 20 noneosinophilic CRSw NP patients.Immunostaining and western blot were used to examine protein expression of EP receptors.Immunohistochemical staining of consecutive serial sections was conducted between EP receptor and eosinophil cationic protein(ECP).m RNA expression of EP,IL-5 and IL-13 were examined by means of quantitative RT-PCR.Results:EP receptors were expressed mainly on epithelium,glands and infiltrating inflammatory cells in nasal tissue.EP1 receptor is selectively expressed in subepithelial inflammatory cells.Compared with the controls and non-ECRSwNP group,the m RNA and protein expressions of EP1 were up-regulated in the ECRSw NP group,but there was no significant difference in the expression of EP2,EP3 and EP4 receptors among the three groups.Immunohistochemical staining of serial sections showed that EP1-positive eosinophils accounted for 50%of the total EP1-positive cells.EP1 m RNA in polyp tissue was positively correlated with IL-5(r=0.55;P<0.001)and IL-13(r=0.69;P<0.001)m RNA expression levels.Conclusions:The up-regulation of EP1 expression in ECRSw NP maybe associated with excessive infiltrations of eosinophils and other inflammatory cells.EP1 receptor may participate in the occurrence and development of ECRSw NP tissue inflammation through chemotaxis and activation of eosinophils.
