降脂灵颗粒治疗HIV/AIDS患者高效抗逆转录病毒治疗后血脂异常随机对照研究

摘　　要：观察降脂灵颗粒治疗经高效抗逆转录病毒疗法(HAART)后血脂异常的脾肾两虚兼痰瘀结滞证HIV/AIDS患者临床疗效及安全性。方法将50例HAART后血脂异常的HIV/AIDS患者按随机数字表分为中药组和对照组,每组25例。中药组予HAART加降脂灵颗粒,对照组予HAART加非诺贝特胶囊。在基线、治疗12、24周、停药12、24周时比较两组血脂[包括TC、TG、HDL-C、LDL-C]、颈动脉内膜中层厚度(IMT)、肝、肾功能[包括ALT、AST、eGFR]、CD4^(+)T和CD8^(+)T淋巴细胞数及HIV-1-RNA水平。结果两组最终纳入病例共48例,中药组25例,对照组23例。与本组基线比较,两组治疗12周时TG下降,且对照组较中药组下降明显(P<0.05)。治疗24周时,中药组HDL-C较基线升高(P<0.05)。中药组各时间点IMT较基线明显下降(P<0.01),且明显低于对照组(P<0.01)。中药组各时间点eGFR明显高于本组基线及对照组(P<0.01)。结论降脂灵颗粒可降低经HAART后HIV/AIDS患者TG、IMT并升高HDL-C,且提高eGFR。Objective To observe the clinical efficacy and safety of Jiangzhiling Granules in treatment of Dyslipidemia caused by highly active antiretroviral therapy(HAART)on deficiency of Pi and Shen combined with stagnation of phlegm and blood stasis HIV/AIDS patients.Methods Totally 50 HIV/AIDS patients were assigned to 2 groups,25 cases in Chinese medicine(CM)group and 25 in control group.The patients in CM group received Jiangzhiling Granules combined with HAART,and the control group received Fenofibrate capsules combined with HAART.The results of blood lipid(including TC,TG,HDL-C,LDL-C),carotid intima-media thickness(IMT),liver and kidney function(including ALT,AST,eGFR),CD4^(+)T and CD8^(+)T lymphocyte count and HIV-1-RNA were compared at 5 time points:baseline,12,24 weeks after treatment,and 12,24 weeks after drug withdrawal.Results Totally 48 cases were included in the 2 groups,25 cases in CM group and 23 cases in control group.Compared with baseline,the level of TG decreased in both groups after 12 weeks of treatment,and significant decrease occurred in control group compared with CM group(P<0.05).After 24 weeks of treatment,HDL-C in CM group was higher than baseline(P<0.05).Compared with baseline and control group,the IMT of CM group decreased at every time point(P<0.01).Meanwhile,the level of eGFR in CM group increased all the time(P<0.01).Conclusion Jiangzhiling Granules could reduce TG,IMT,increase HDL-C and eGFR of HIV/AIDS patients after HAART.
