摘　　要：目的分析Allium覆膜金属输尿管支架在恶性肿瘤所致复杂输尿管梗阻患者中的临床应用价值。方法回顾性分析2018年10月至2020年10月青岛市海慈医疗集团收治的37例恶性肿瘤所致复杂输尿管梗阻患者的临床资料。患者行肾镜或输尿管镜下Allium覆膜金属输尿管支架置入治疗。记录患者治疗情况、并发症发生情况,并对比患者治疗前后血肌酐、尿素氮、肾小球滤过率、血红蛋白变化。结果顺行与逆行置入Allium覆膜金属输尿管支架患者的手术时间、术中出血量比较差异均无统计学意义(均P>0.05)。术后共有3例患者发生并发症,其中发热2例、腰痛1例,均经保守治疗后好转。术后2个月和6个月,患者肌酐水平均显著低于术前、术后1周和术后2周(均P<0.05)。术后1周至术后6个月,患者尿素氮水平均显著低于术前(均P<0.05),肾小球滤过率均显著高于术前(均P<0.05)。结论Allium覆膜金属输尿管支架置入在恶性肿瘤所致复杂输尿管梗阻患者中的效果确切、安全性高,能够显著改善患者肾功能。Objective To analyze the clinical application value of Allium covered metal ureteral stent in patients with complex ureteral obstruction caused by malignant tumor.Method The clinical data of 37 patients with complex ureteral obstruction caused by malignant tumor admitted to Qingdao Hiser Hospital from October 2018 to October 2020 were retrospectively analyzed.Patients were treated with Allium covered metal ureteral stent under nephroscope or ureteroscope.The treatment and complications were recorded,and the changes of serum creatinine,urea nitrogen,glomerular filtration rate and hemoglobin before and after treatment were compared.Result There were no significant differences in operation time or intraoperative bleeding volume between the anterograde and retrograde patients with Allium covered metal ureteral stent(all P>0.05).Complications occurred in 3 patients postoperatively,including fever in 2 cases and lumbago in 1 case,all of which were improved after conservative treatment.The creatinine levels of the patients were significantly lower than those before operation,1 week and 2 weeks after operation(all P<0.05).The urea nitrogen levels from 1 week to 6 months after operation were significantly lower than those before operation(all P<0.05),and the glomerular filtration rate was significantly higher than that before operation(all P<0.05).Conclusion Allium covered metal ureteral stent implantation in patients with complicated ureteral obstruction caused by malignant tumor is effective and safe,and can significantly improve the renal function of patients.
