17例足跗骨慢性骨髓炎的治疗经验

摘　　要：目的:探讨足跗骨慢性骨髓炎的治疗效果。方法:回顾性研究分析2017年1月-2019年10月我院创伤骨科收治的17例足跗骨慢性骨髓炎患者临床资料,骨髓炎病程最长5年,最短4个月,平均2年。所有患者均采用“蛋壳式”彻底清创后,用载敏感抗生素硫酸钙填充骨缺损。结果:本组慢性骨髓炎1期治愈率达到94%(16/17),所有患者得到有效随访,最长24个月,最短6个月,平均18个月。术后随访X线提示硫酸钙逐步吸收并诱导成骨良好。根据美国足踝外科协会后踝-后足功能评分(AOFAS评分)评价结果,13例优,4例良。结论:采用蛋壳式彻底清创加载敏感抗生素的硫酸钙骨腔填充方法,可以有效的治疗足跗骨慢性骨髓炎,对于足跗骨的骨感染疮疗,具有一定的临床意义。Objective:Discuss the clinical therapeutic effect of Chronic osteomyelilis of the fooL larsal.Methods:Retrospective analysis Between Jan,2017 and Oct,2019,Lhe Orthopaedic Trauma from our hospital has gol 17 patients who suffered from foot tarsal chronic osteomyelitis.Osteomyelitis lasts 2 years by average l^etween the longest 5 year9 and the shortest 4 months.All patients have been filled the damaged bone by carrier sensitive antibiotic calcium sulfate after the process of extensive eggshell-like debridement.Results:All patients have got valid following checks by average 18 months between Lhe longest 24 months and Lhe shortest 6 months.The X-ray shows that the Calcium suKate is absorbed term by term and gives a positive feedback to the boners recovery after the surgery.The cure rate of osteomyelitis reaches to 94%(16/17)in lhe first period.It Lurns to 13 good levels and 4 fine levels according to AOFAS.Conclusion:To HU lhe bone by carrier sensitive antibiotic calcium sulfate after lhe process of extensive eggshell-like debridement is a valid way to Ireat chronic osteomyelitis of the foot tarsal.It conforms to the therapeutic rules of chronic osteomyelitis,and has got positive meanings to the clinical therapeutic treatment of the foot larsal.
