乙酰半胱氨酸及吡非尼酮联合泼尼松治疗特发性肺纤维化的效果

摘　　要：目的探讨乙酰半胱氨酸、吡非尼酮联合泼尼松对特发性肺纤维化(IPF)患者肺功能及肺间质纤维化指标的影响。方法采用前瞻性随机试验方法,选取2018年1月-2020年2月于医院接受治疗的85例IPF患者为研究对象,按随机数字表法分为对照组(42例)与观察组(43例)。对照组采用吡非尼酮+泼尼松治疗,观察组在对照组基础上联合乙酰半胱氨酸治疗。2组均连续治疗6个月,对比2组治疗前、治疗6个月后肺功能及肺间质纤维化指标。结果治疗6个月后,2组一氧化碳弥散量(DLco)、动脉血氧分压(PaO;)水平均较治疗前升高,且观察组高于对照组,差异有统计学意义(P<0.05);治疗6个月后,2组透明质酸(HA)、Ⅲ型胶原(Ⅲ-C)、Ⅳ型胶原(Ⅳ-C)水平均较治疗前降低,且观察组低于对照组,差异有统计学意义(P<0.05)。结论 IPF患者采用乙酰半胱氨酸、吡非尼酮联合泼尼松治疗可有效改善患者肺功能及肺间质纤维化指标。Objective To investigate the effects of acetylcysteine,pirfenidone combined with prednisone on pulmonary function and pulmonary interstitial fibrosis indexes in patients with idiopathic pulmonary fibrosis(IPF).Methods A prospective randomized trial was conducted in 85 patients with IPF who received hospital treatment from January 2018 to February 2020. According to random number table method,they were divided into control group(42 cases) and observation group(43 cases). The control group was treated with pirfenidone + prednisone,and the observation group was treated with acetyl cysteine on the basis of the control group. The lung function and interstitial fibrosis indexes of the two groups were compared before and after treatment for 6 months. Results After 6 months of treatment,the levels of carbon monoxide dispersion(DLco) and arterial partial pressure of oxygen(PaO;) in 2 groups were higher than those before treatment,and the observation group was higher than the control group,the differences were statistically significant(P <0.05). After 6 months of treatment,the levels of hyaluronic acid(HA),type Ⅲ collagen(Ⅲ-C) and type Ⅳ collagen(Ⅳ-C) in both groups were lower than those before treatment,and the observation group was lower than the control group,the difference was statistically significant(P<0.05). Conclusion Acetylcysteine and pirfenidone combined with prednisone can effectively improve the pulmonary function and pulmonary interstitial fibrosis indexes in IPF patients.
