摘 要:术后谵妄(POD)是麻醉和手术后常见的并发症之一,其与患者预后不良及病死率的增加相关。围手术期维持良好的组织灌注和脑供氧是改善POD的重要措施,而合适的液体治疗可优化脑组织微循环,改善脑组织灌注。因此,围手术期合适的液体治疗对降低POD发生率具有重要意义。目前国内外研究发现优化术前液体管理、术中液体管理(包括目标导向输液、合适的输液类型和输液量)可以改善脑灌注及脑供氧,从而减少POD的发生率,而关于术后液体管理对POD影响的研究,未来还需要更大样本的临床研究来论证。Postoperative delirium(POD)is one of common complications after anesthesia and surgery,which is associated with poor prognosis and increased mortality.Maintaining good perioperative tissue perfusion and cerebral oxygen supply are important measures to improve POD,while appropriate fluid management can optimize cerebral microcirculation and improve cerebral perfusion.Therefore,appropriate perioperative fluid management during perioperative has important clinical significance to reduce the incidence of POD.At present,studies at home and abroad have found that optimizing preoperative and intraoperative fluid management(including goal-directed fluid therapy,appropriate infusion type and infusion volume)can improve cerebral perfusion and cerebral oxygen supply,thereby reducing the incidence of POD.However,there are few studies on the impact of postoperative fluid management on POD.Clinical studies with larger samples are needed in the future to demonstrate the impact of optimizing perioperative fluid management on POD.
