摘　　要：目的探讨重症肺炎患者血清及肺泡灌洗液(BALF)C反应蛋白(CRP)、降钙素原(PCT)及白细胞介素-6(IL-6)的表达水平及其临床价值。方法回顾性分析2019年10月至2020年10月该院重症监护病房收治的108例肺部感染患者的临床资料,根据其肺部感染严重程度分为重症肺炎组(51例)和肺炎组(57例),分别测定2组患者血清及BALF CRP、PCT、IL-6水平,使用受试者工作特征曲线评估血清及BALF CRP、PCT、IL-6对重症肺炎的早期诊断价值。结果重症肺炎组患者血清及BALF CRP、PCT、IL-6水平均明显高于肺炎组,差异均有统计学意义(P<0.001);血清CRP、PCT及BALF IL-6对早期重症肺炎均具有较高的诊断效能(曲线下面积=0.975、0.948、0.993),其截断值分别为134.5 mg/L、4.5 ng/mL、389 pg/mL。结论血清CRP、PCT及BALF IL-6对重症肺炎的早期诊断具有重要参考意义。Objective To explore the expression and clinical value of C-reactive protein(CRP),procalcitonin(PCT)and interleukin-6(IL-6)in serum and bronchoalveolar fluid(BALF)of patients with severe pneumonia.Methods A retrospective analysis was conducted on the clinical data of 108 patients with pulmonary infection in the Department of Critical Care Medicine of this hospital from October 2019 to October 2020.According to the severity of pulmonary infection,the patients were divided into the severe pneumonia group(51 cases)and the pneumonia group(57 cases).The levels of CRP,PCT and IL-6 in serum and BALF were measured in the two groups.The receiver-operating characteristic curve was used to evaluate the early diagnosis value of CRP,PCT and IL-6 in serum and BALF on the severe pneumonia.Results The levels of CRP,PCT and IL-6 in serum and BALF in the severe pneumonia group were higher than those in the pneumonia group(P<0.001).The CRP,PCT values in serum and IL-6 value in BALF had high diagnostic efficiency in the early severe pneumonia(area under curve=0.975,0.948,0.993),and the cut-off values were 134.5 mg/L,4.5 ng/mL,389 pg/mL,respectively.Conclusion The CRP,PCT in the serum and IL-6 in the BALF have important reference value in the early diagnosis of severe pneumonia.
