摘　　要：复发性流产和辅助生殖技术中的反复种植失败与免疫因素密切相关,目前比较有共识的相关免疫因素有:自身抗体异常、细胞免疫异常及各种细胞因子异常表达等。对免疫系统的调节和抑制作用在其他领域的应用发展迅速,某些免疫制剂如糖皮质激素、环孢素、羟氯喹,免疫疗法如宫腔内注射自体外周血单个核细胞、静脉注射用人免疫球蛋白等也被应用于复发性流产和反复种植失败的治疗,这些治疗方法被证实可能对改善患者妊娠结局有明显或潜在益处,因此研究免疫治疗对复发性流产及反复种植失败的作用机制、有效性及安全性等对复发性流产和反复种植失败患者有重要意义。本文将从复发性流产及反复种植失败的免疫治疗出发,探讨其作用机制、临床应用、安全性等。Recurrent spontaneous abortion and repeated implantation failure in assisted reproductive technology are closely related to immune factors.At present,there is a consensus that related immune factors include abnormal autoantibodies,abnormal cellular immunity and abnormal expression of various cytokines.The regulation and inhibition of the immune system have been rapidly applied in other fields,some immune agents such as corticosteroids,cyclosporin A and hydroxychloroquine,and immune therapy such as intrauterine injection of autologous peripheral blood mononuclear cells intravenous immunoglobulin,have also been used in the treatment of recurrent spontaneous abortion and repeated implantation failure,and these treatments may prove to improve the pregnancy outcome in patients with a significant or potential benefits.Therefore,it is of great significance to study the mechanism of action,efficacy and safety of immunotherapy in the treatment of recurrent spontaneous abortion and repeated implantation failure.This paper will discuss the mechanism of action,clinical application and safety of immunotherapy in recurrent spontaneous abortion and repeated implantation failure.
