基于吴门“清/下法”的三黄胶囊对2型糖尿病便秘患者血糖水平及肠道菌群的影响研究

摘　　要：目的:探讨基于吴门“清/下法”的苏州市中医医院院内制剂三黄胶囊对2型糖尿病便秘患者血糖水平及肠道菌群的影响。方法:将60例符合湿热型2型糖尿病合并便秘患者根据随机数字表法随机分为治疗组和对照组各30例。另从苏州市中医医院体检中心选取30例健康志愿者作为健康组。对照组给予常规降糖药物治疗,治疗组在对照组基础上加用三黄胶囊。两组疗程均为8周。观察两组患者治疗前后的中医证候疗效、便秘临床症状评分、Bristol评分、空腹血糖、餐后2小时血糖、糖化血红蛋白及肠道菌群的变化情况。结果:治疗组中医证候疗效显著优于对照组(P<0.05);治疗组治疗后空腹血糖与治疗前无显著差异(P>0.05),2小时血糖与糖化血红蛋白较治疗前显著改善(P<0.05);治疗组治疗后双歧杆菌、拟杆菌、乳杆菌数量均较治疗前明显升高,肠杆菌、肠球菌、酵母菌数量均较治疗前显著降低(P<0.05)。结论:三黄胶囊能缓解2型糖尿病合并便秘患者的便秘状态,改善血糖水平,调整肠道菌群,为从“脾”、从“湿”论治2型糖尿病提供新思路。Objective:To explore the effects of the Sanhuang capsules(三黄胶囊)based on Wu’s Qing/Xia therapy(吴门“清/下法”)on blood glucose level and intestinal microflora in type 2 diabetic mellitus patients with constipation.Methods:60 cases were randomly divided into the treatment group and the control group,30 cases for each.In addition,30 healthy volunteers were selected from the physical examination center and were divided into the healthy group.The control group was given routine hypoglycemic medicines,and the treatment group was given the Sanhuang capsules more.The treatment course for both groups was 8 weeks.Results:The clinical efficacy of TCM syndromes in the treatment group was better than the control group.The GLU level in the treatment group after treatment was not significantly different from that before treatment(P>0.05).2 h PBG and HbA1c levels in the treatment group after treatment were significantly improved compared with those before treatment(P<0.05).In the treatment group,the number of bifidobacteria,bacteroidetes and lactobacillus was increased significantly after treatment,while the number of enterobacteria,enterococcus and yeast was decreased significantly after treatment(P<0.05).Conclusion:The Sanhuang capsule can relieve constipation state of type 2 diabetic mellitus patients with constipation,ameliorated glucose level and regulated intestinal microflora.It provided a new idea for the treatment of type 2 diabetic mellitus.
