经筋推拿联合中药热敷治疗血瘀型腰椎间盘突出症的临床研究

摘　　要：目的:探究经筋推拿联合中药热敷治疗血瘀型腰椎间盘突出症的临床疗效。方法:选择2018年3月-2020年3月在无锡市中医医院就诊的50例血瘀型腰椎间盘突出症患者作为研究对象,根据随机抽签原则分为对照组(采用经筋推拿治疗)和试验组(在对照组经筋推拿治疗的基础上联用中药热敷治疗),每组25例,观察两组治疗后的临床疗效,治疗前后两组中医证候评分,疼痛视觉模拟评分法(Visual Analog Scale,VAS)评分以及Oswestry功能障碍指数(Oswestry Disability Index,ODI)。结果:试验组的临床疗效有效率比对照组高(P<0.05)。两组治疗前1 d的VAS评分、中医证候评分对比差异不显著(P>0.05)。两组治疗后3 d,VAS评分、中医证候评分均低于治疗前1 d(P<0.05),且试验组的VAS评分、中医证候评分均比对照组低(P<0.05)。治疗前1 d,两组的ODI评分对比无显著性差异(P>0.05);治疗后3 d,两组的ODI评分比治疗前1 d低(P<0.05)。结论:采用中药热敷联合经筋推拿治疗血瘀型腰椎间盘突出症可明显改善临床症状,提高临床疗效有效率,降低中医证候评分,减轻疼痛,改善腰椎功能。Objective:The clinical effects of tendon massage plus TCM medicine hot compress on the blood stasis type lumbar intervertebral disc herniation were explored.Methods:A total of 50 cases from March 2018 to March 2020 were selected as the research objects,and were divided into the control group(took tendon massage)and the experimental group(received tendon massage plus TCM medicine hot compress)according to random drawing,25 cases in each.Results:The clinical efficacy in the experimental group was higher than the control group(P<0.05).There was no significant difference in VAS score and TCM syndrome score between the groups on 1 day before treatment(P>0.05).On 3 days after treatment,the VAS score and TCM syndrome score in the groups were lower than those before treatment(P<0.05),and in the experimental group were lower than the control group(P<0.05).There was no significant difference in ODI between the groups on 1 day before treatment(P>0.05);on 3 days after treatment,the ODI in the groups were lower than before treatment(P<0.05),and in the experimental group were lower than the control group(P<0.05).Conclusion:Treating the blood stasis type lumbar intervertebral disc herniation by tendon massage plus TCM medicine hot compress can significantly ameliorate the clinical symptoms,increase the clinical efficacy,reduce the patient’s TCM syndrome score,relieve pain,and improved lumbar function.
