热敏灸配合双氯芬酸钾预防全髋关节置换术后异位骨化临床观察

摘　　要：目的:探讨热敏灸配合双氯芬酸钾片对降低全髋关节置换术后异位骨化发生率的临床效果。方法:抽取2015年1月-2018年12月在北京中医药大学枣庄医院进行全髋关节置换术的患者共计149例进行观察,采用热敏灸配合口服双氯芬酸钾片的方案预防全髋关节置换术后异位骨化,本组患者设置为观察组,另选择同一时期在北京中医药大学枣庄医院进行全髋关节置换术的患者149例作为对照组,本组患者术后采用常规方案及口服双氯芬酸钾片预防全髋关节置换术后异位骨化。比较两组患者治疗结束后的异位骨化率以及并发症的发生率。结果:热敏灸配合双氯芬酸钾片预防全髋关节置换术后异位骨化发生率(RR=0.09;95%CI 0.03~0.26)及胃肠道并发症发生率均较口服单纯双氯芬酸钾片更低,P<0.05;干预后观察组患者疼痛数字分级法(Numeric Rating Scales,NRS)评分低于干预前和对照组(P<0.05)。干预前,两组患者的T细胞亚群CD_(3)^(+)、CD_(4)^(+)、CD_(8)^(+)含量的差异无统计学意义(P>0.05),经过干预,两组患者T细胞亚群CD_(3)^(+)、CD_(4)^(+)、CD_(8)^(+)含量升高,观察组显著高于对照组(P<0.05)。结论:热敏灸配合双氯芬酸钾片能有效降低全髋关节置换术后异位骨化的发生率,值得推广应用。Objective:To investigate the clinical efficacy of thermal moxibustion plus diclofenac potassium tablets on reducing the incidence of heterotopic ossification after total hip replacement.Methods:From January 2015 to December 2018,149 cases in the observation group were given thermal moxibustion plus diclofenac potassium tablets.The 149 patients in the control group were given conventional treatment plus oral diclofenac potassium tablets.Results:The incidence of heterotopic ossification after total hip replacement(RR=0.09;95%CI 0.03~0.26)and the incidence of gastrointestinal complications in the observation group were lower,P<0.05.After the intervention,the NRS scores in the observation group were lower than the pre-intervention and the control group(P<0.05).The levels of T cell subsets CD_(3)^(+),CD_(4)^(+),and CD_(8)^(+)in the two groups were increased,and in the observation group were significantly higher(P<0.05).Conclusion:Thermal moxibustion plus diclofenac potassium tablets can effectively reduce the incidence of heterotopic ossification after total hip arthroplasty,and it is worthy of popularization and application.
