摘 要:目的探究抢救室中无创通气治疗急性左心衰竭的临床效果及对患者血压与心率水平的影响。方法选取2019年1月至2020年3月于本院急诊科进行抢救的60例急性左心衰竭患者作为研究对象,随机分为对照组与观察组,每组30例。对照组采用常规抢救措施,观察组在对照组基础上采用无创通气治疗,比较两组治疗前及治疗后1、4 h的血压、心率、呼吸频率、血气分析指标和治疗效果。结果观察组治疗总有效率为96.67%,明显高于对照组的70.00%,差异有统计学意义(P<0.05)。治疗前,两组收缩压、舒张压、呼吸频率、心率比较差异无统计学意义;治疗后1、4 h,观察组收缩压、舒张压、呼吸频率、心率均低于对照组(P<0.05)。治疗前,两组血气指标比较差异无统计学意义;治疗后1、4 h,观察组PaO_(2)、SaO_(2)均高于对照组,PaCO_(2)均低于对照组,差异有统计学意义(P<0.05)。结论在急性左心衰竭患者的急救过程中采用无创通气的临床应用效果显著,可明显改善患者的呼吸功能,维持患者的血压及血氧平衡,值得临床推广应用。Objective To explore the clinical effect of non-invasive ventilation in the emergency room in the treatment of acute left heart failure and its influence on the blood pressure and heart rate of patients.Methods 60 patients with acute left heart failure who were rescued in the emergency department of our hospital from January 2019 to March 2020 were selected as the research subjects,and they were randomly divided into control group and observation group,with 30 cases in each group.The control group was treated with conventional rescue measures,and the observation group was treated with non-invasive ventilation on the basis of the control group.The blood pressure,heart rate,respiratory rate,blood gas analysis indexes before treatment and 1,4 h after treatment and treatment effects of the two groups were compared.Results The total effective rate of treatment in the observation group was 96.67%,which was significantly higher than 70.00%in the control group,and the difference was statistically significant(P<0.05).Before treatment,there was no statistically significant difference in systolic blood pressure,diastolic blood pressure,respiratory frequency,and heart rate between the two groups;1,4 h after treatment,the systolic blood pressure,diastolic blood pressure,respiratory frequency,and heart rate of the observation group were lower than those of the control group(P<0.05).Before treatment,there was no statistically significant difference in blood gas indexes between the two groups;1,4 h after treatment,PaO_(2) and SaO_(2) in the observation group were higher than those in the control group,and PaCO_(2) was lower than the control group,the difference was statistically significant(P<0.05).Conclusion The clinical application of non-invasive ventilation in the emergency process of patients with acute left heart failure is significant.It can significantly improve the patient's respiratory function and maintain the patient's blood pressure and blood oxygen balance.It is worthy of clinical application.
