盆底超声参数与SUI患者肌纤维疲劳度和病情程度的相关性分析

摘　　要：目的探讨盆底超声参数与压力性尿失禁(SUI)患者病情程度、肌纤维疲劳度的相关性。方法选取2018年10月至2020年3月该院SUI患者146例为研究对象,均接受盆底超声检查并给予盆底肌训练。分析盆底超声参数[膀胱颈旋转角度(θ)、膀胱颈下移距离(X)、膀胱颈移动度(Mu)、膀胱尿道后角(α)]与SUI问卷评分(SS)、肌纤维疲劳度的相关性,以及分析其预测SUI疗效不佳的价值。结果不同病情程度患者年龄、受教育程度、年收入、分娩方式、会阴情况、流产次数和X比较差异无统计学意义(P>0.05),BMI、既往分娩次数、新生儿体重、θ、Mu、α比较差异有统计学意义(P<0.05)。SUI患者X与SS、盆底肌纤维疲劳度无显著相关性(P>0.05),θ、Mu、α与SS评分、盆底肌纤维疲劳度呈正相关(P<0.05)。疗效良好者θ、Mu、α低于疗效不佳者(P<0.05);θ预测的曲线下面积(AUC)最大,临界值37.35°时,其灵敏度、特异度分别为94.74%、59.84%。结论盆底超声参数中θ、Mu、α与SUI患者肌纤维疲劳度、病情程度存在良好线性关系。Objective To explore the correlation between the pelvic floor ultrasound parameters with the disease severity and muscle fiber fatigue in the patients with stress urinary incontinence(SUI).Methods A total of 146 patients with SUI in this hospital from October 2018 to March 2020 were selected as the study subjects.The correlation between the pelvic floor ultrasound parameters[angle of bladder neck rotation(θ),distance of bladder neck down(X),degree of bladder neck movement(Mu)and posterior angle of bladder urethra(α)]with the SUI questionnaire score(SS)and muscle fiber fatigue was analyzed,and the value of pelvic floor ultrasound parameters for predicting the poor efficacy of SUI was also analyzed.Results The age,educational degree,annual income,delivery mode,perineal condition,abortion times and X had no statistical difference among the patients with different disease severities(P>0.05).BMI,previous delivery times,newborns body mass,θ,Mu andα,theθhad statistical difference(P<0.05);X in the patients with SUI had no significant correlation with the SS and pelvic floor muscle fiber fatigue(P>0.05).θ,Mu andαlevels were positively correlated with the pelvic floor muscle fiber fatigue(P<0.05).θ,Mu andαlevels in the patients with good effect were lower than those in the patients with poor effect(P<0.05);AUC ofθprediction was maximal,when theθcritical value was 37.35°,its sensitivity and specificity were 94.74%and 59.84%respectively.Conclusion Among the pelvic floor ultrasound parameters,θ,Mu andαhave the good linear relation with the muscle fiber fatigue and disease severity in the patients with SUI.
