孕11~13^(+6)周胎儿超声大脑中动脉峰值流速、胎盘体积、胎盘厚度、心胸横径比与顶臀长的相关性

摘　　要：目的研究孕11~13^(+6)周胎儿超声大脑中动脉峰值流速(MCA PSV)、胎盘体积(PV)、胎盘厚度(PT)、心胸横径比(CTR)与顶臀长(CRL)的相关性,建立早孕期上述指标正常参考值范围。资料与方法测量212名11~13^(+6)周正常胎儿MCA PSV、PV、PT、CTR及CRL,按照CRL的大小分为11周、12周、13周3个组,每组分别31名、119名、62名,采用Pearson相关分析上述指标与CRL的相关性,并建立不同孕周上述指标的正常参考值范围。结果孕11~13^(+6)周,胎儿MCA PSV、PV、PT均与CRL呈正相关(r=0.389、0.434、0.313,P均<0.001);CTR与CRL无相关性(r=0.135,P=0.057)。11周、12周和13周组MCA PSV参考值(95%CI)分别为8.22~18.46 cm/s、10.81~21.83 cm/s、11.92~22.70 cm/s,PV分别为19.15~48.97 cm^(3)、20.80~69.99 cm^(3)、26.24~79.36 cm^(3),PT分别为9.53~18.35 mm、10.08~19.73 mm、10.18~22.52 mm,CTR分别为0.37~0.41、0.35~0.43、0.34~0.42。结论孕11~13^(+6)周胎儿MCA PSV、PV、PT随着CRL增长而增大,CTR不随CRL的变化而改变。本研究建立了MCA PSV、PV、PT、CTR不同孕周的正常参考值范围。Purpose To study the correlation between fetal middle cerebral artery peak velocity(MCA PSV),placental volume(PV),placental thickness(PT),cardio thoracic ratio(CTR)and crown-rump length(CRL)in 11-13^(+6) weeks of gestation,and to establish their reference ranges in early pregnancy.Materials and Methods A total of 212 fetuses were selected,the MCA PSV,PV,PT,CTR and CRL were measured,and all cases were further divided into three groups according to CRL,including 31 cases with 11 weeks of gestation group,119 cases with 12 weeks of gestation group,and 62 cases with 13 weeks of gestation group.Pearson correlation was performed to analyze the correlation between these indicators and CRL,establishing the reference ranges of these indicators during 11-13^(+6) weeks of gestation.Results During 11-13^(+6) weeks of gestation,PSV,PV and PT of fetal MCA were positively correlated with CRL(r=0.389,0.434,0.313,P<0.001).No correlation between CTR and CRL was found(r=0.135,P=0.057).The reference ranges of the indexes of 11 weeks group,12 weeks group and 13 weeks group were as follows:MCA PSV was 8.22-18.46 cm/s,10.81-21.83 cm/s,11.92-22.70 cm/s;PV was 19.15-48.97 cm^(3),20.80-69.99 cm^(3),26.24-79.36 cm^(3);PT was 9.53-18.35 mm,10.08-19.73 mm,10.18-22.52 mm;CTR was 0.37-0.41,0.35-0.43,0.34-0.42,respectively.Conclusion PSV,PV and PT of MCA in fetuses at 11-13^(+6) weeks of gestation increased with the increase of CRL,however,the CTR did not change with the change of CRL.The normal reference ranges of MCA PSV,PV,PT and CTR at different gestational weeks are initially established.
