缺血性脑卒中患者心理一致感与反刍性沉思、家庭关怀度的关系研究

摘　　要：目的了解缺血性脑卒中患者的心理一致感、反刍性沉思及家庭关怀度现状,探讨反刍性沉思及家庭关怀度对心理一致感的影响,为提高患者心理一致感水平,改善心理健康状况提供理论依据。方法选取2019年3月至2020年6月在四川省某三级甲等医院住院治疗的295例缺血性脑卒中患者作为研究对象,采用一般资料调查表、心理一致感量表、中文版事件相关反刍性沉思问卷、家庭关怀度指数问卷对其进行问卷调查。结果缺血性脑卒中患者心理一致感总分为(61.86±8.43)分,反刍性沉思总分为(29.77±8.32)分,家庭关怀度总分为(7.12±1.84)分。缺血性脑卒中患者心理一致感与反刍性沉思及家庭关怀度呈正相关(r=0.471,P<0.01;r=0.686,P<0.01);多元线性回归分析结果显示,缺血性脑卒中患者心理一致感与文化程度、家庭人均月收入、患者入院就诊时间、自理能力、反刍性沉思、家庭关怀度有关,可联合解释心理一致感44.1%的总变异。结论缺血性脑卒中患者心理一致感处于偏低水平,医护工作者应重视心理一致感的影响因素,有意识地发现并有效识别患者的心理一致感状态,引导患者感知在疾病应对过程中获得的社会支持,帮助患者建立支持系统,以此提高其心理健康水平。Objective To investigate the status quo of sense of congruence,ruminant meditation and family care in patients with ischemic stroke,and the influence of ruminant meditation and family care on psychological congruence,so as to provide theoretical basis for improving the level of sense of congruence and mental health status of patients.Methods A total of 295 ischemic stroke patients hospitalized from March 2019 to June 2020 in a 3A-level hospital were investigated with the general information questionaire,Sense of Coherence-13,the Chinese Version of Event Related Rumination Inventory,and the familiy APGAR index.Results The total score of sense of coherence in patients with ischemic stroke was(61.86±8.43).The total score of rumination meditation was(29.77±8.32)and the total score of family care was(7.12±1.84).The sense of coherence was positively correlated with rumination meditation and family care(r=0.471,P<0.05,r=0.686,P<0.01).Multiple linear regression analysis showed that the sense of coherence in patients with ischemic stroke was associated with education level,family monthly income per capita,time from onset to hospitalization,self-care ability,ruminant meditation and family care,which accounted for 44.1%of the total variability.Conclusion Patients with ischemic stroke has a lower middle sense of coherence level.Health care workers should pay attention to the influencing factors of sense of coherence,consciously discover and effectively identify the patient's sense of coherence state,guide patients to perceive the social support they received in the course of disease response,and help patients establish a support system to improve their mental health.
