摘 要:目的探讨超声心动图在妊娠期糖尿病(GDM)胎儿心脏构型及心功能评价中的应用价值。方法回顾性分析于2019年1月至2021年1月期间在连云港市第二人民医院行产检的80例孕妇资料,其中38例孕妇确诊为GDM,设为研究组;余42例为正常孕妇,设为对照组。于2组孕妇妊娠末期进行超声心动图检测。观察比较2组胎儿的室间隔舒张末期以及收缩期厚度(IVSd、IVSs),同时比较胎儿左右室壁厚度(LVWT、RVWT)、左右心室舒张末期内径(LVDd、RVDd)、左右心室收缩末期内径(LVDS、RVDS),计算2组胎儿的心脏二尖瓣E、A峰值流速比(E/AMV)、胎儿心脏Tei指数(MPI)以及心脏三尖瓣E、A峰值流速比(E/ATV)。结果2组胎儿的RVDd、LVDd、RVDS以及LVDS比较,差异无统计学意义(P>0.05);研究组胎儿的E/ATV、E/AMV为0.47±0.02、0.50±0.04,明显低于对照组(0.90±0.10,0.92±0.15),但IVSs、IVSd、LVWT、RVWT为(5.49±1.42)、(5.45±1.23)、(5.21±1.33)、(5.22±1.57)mm,均显著高于对照组[(2.71±1.16)、(3.02±0.84)、(3.18±1.14)、(4.03±1.15)mm],差异均有统计学意义(P<0.05)。结论妊娠期糖尿病会对胎儿的心脏构型产生一定影响进而影响胎儿心脏发育和心功能,应用超声心动图检测妊娠期糖尿病胎儿心功能状况可以为临床治疗提供依据。Objective To explore the application value of echocardiography in gestational diabetes mellitus(GDM)fetal heart configuration and cardiac function evaluation.Methods The data of 80 pregnant women who underwent maternity check-ups at the Second People's Hospital of Lianyungang from January 2019 to January 2021 were retrospectively analyzed.Among them,38 pregnant women were diagnosed with GDM and were set as the study group;the remaining 42 cases were normal pregnant women and were set as the control group.Echocardiography was performed at the end of pregnancy in the two groups of pregnant women.The end-diastolic and systolic thickness(IVSd,IVSs)of the fetuses of the two groups were observed and compared,and the fetal left and right ventricular wall thickness(LVWT,RVWT),left and right ventricular end diastolic diameters(LVDd,RVDd),left and right ventricular end systolic diameters(LVDS,RVDS)were compared,the E/A peak velocity ratio(E/AMV),myocardial performance index(MPI)and the cardiac tricuspid valve E/A peak velocity ratio(E/ATV)of the two groups of fetuses were calculated.Results There was no statistically significant difference in RVDd,LVDd,RVDS and LVDS between the two groups of fetuses(P>0.05);The E/ATV and E/AMV of the fetus in the study group were 0.47±0.02,0.50±0.04,which were significantly lower than those in the control group(0.90±0.10 and 0.92±0.15),but the IVSs and IVSd,LVWT,RVWT were(5.49±1.42),(5.45±1.23),(5.21±1.33),(5.22±1.57)mm,which were significantly higher than those in the control group[(2.71±1.16),(3.02±0.84),(3.18±1.14),(4.03±1.15)mm],the differences were statistically significant(P<0.05).Conclusion Gestational diabetes will have a certain impact on the fetal heart configuration and then affect the fetal heart development and heart function.The application of echocardiography to detect the fetal heart function status of gestational diabetes can provide a basis for clinical treatment.
