全髋关节置换术与半髋关节置换术治疗老年股骨颈骨折患者的效果比较

摘　　要：目的:比较全髋关节置换术与半髋关节置换术治疗老年股骨颈骨折患者的效果。方法:选取86例老年股骨颈骨折患者作为研究对象,按随机数字表法将其分为对照组和观察组各43例。对照组行半髋关节置换术治疗,观察组行全髋关节置换术治疗,比较两组手术相关指标水平、术前及术后髋关节功能评分和并发症发生率。结果:观察组术中出血量、总引流量均多于对照组,手术时间长于对照组,差异有统计学意义(P<0.05);观察组Harris髋关节功能评分高于对照组,差异有统计学意义(P<0.05);两组并发症率发生率比较,差异无统计学意义(P<0.05)。结论:全髋关节置换术治疗老年股骨颈骨折中患者提高Harris髋关节功能评分的效果优于半髋关节置换术治疗,但手术相关指标水平同样高于半髋关节置换术治疗。Objective:To compare effects of total hip arthroplasty and hemiarthroplasty in treatment of elderly patients with femoral neck fractures.Methods:86 elderly patients with femoral neck fractures were selected as the research objects,and were divided into control group and observation group according to the random number table method,43 cases in each group.The control group underwent hemi arthroplasty,while the observation group was given total hip arthroplasty.The operation-related indicators,the preoperative and postoperative hip function scores,and the complication rates were compared between the two groups.Results:The intraoperative blood loss,the operation time,and the total drainage level in the observation group were higher than those in the control group,and the differences were statistically significant(P<0.05).The Harris hip function score of the observation group was higher than that of the control group,and the difference was statistically significant(P<0.05).However,there was no statistically significant difference in the incidence of complications between the two groups(P<0.05).Conclusions:Total hip arthroplasty;in the treatment of the elderly patients with femoral neck fractures can improve the Harris hip function score,and the levels of surgery-related indicators are also higher than those of hemiarthroplasty.Moreover,it is superior to hemiarthroplasty.
