社区空巢老人痴呆预防指标体系初步探讨

摘　　要：目的老龄化和家庭小型化致使空巢老人越来越多,空巢老人不良生活方式和心理问题使得发生痴呆的概率增加。本研究初步探讨社区空巢老人痴呆预防的指标体系,为预防老年痴呆和社区健康管理人员进行痴呆干预提供参考。方法2018-07-03-2018-12-05在进行大量文献和访谈研究基础上,形成空巢老人痴呆预防的指标体系调查问卷。采取便利抽样法,选取30位来自于医院和企业的专家。应用Likert 5级评分法评判问卷条目重要性,并进行变异性分析,2轮专家函询后初步形成社区空巢老人痴呆预防的指标体系。结果2轮咨询发放专家问卷30份,回收30份,回收率为100%,回收问卷均有效。专家权威程度熟悉系数0.79,判断系数0.83,权威系数0.78。Cronbanch’s a系数为0.83。第1轮专家函询后,32项条目重要性评分为3.32~4.86分,变异系数为2.48%~5.06%,以重要性均值>3.5、变异系数<20%的指标作为进入第2轮咨询的标准。2轮咨询后,问卷条目共增加4条,修改7条。第2轮专家函询后,36项条目的重要性评分为3.42~4.86分,变异系数为2.96%~6.64%。结论通过2轮专家函询初步建立了社区空巢老人痴呆预防的指标体系,为空巢老人社区干预提供参考,后期需实践验证指标体系并进一步修正完善。Objective To construct Indicator system of dementia prevention for empty nest elderly in community and provide reference for disease prevention and dementia intervention by community health managers.Methods A large number of literatures and interviews were conducted by the research group,and a questionnaire on the first-level preventive measures of empty-nester dementia was initially formed during July 10,2018 to December 15,2018.A convenient sampling method was used to select 30 experts from relevant fields.The importance of the five-level Likert score items was applied and the variability was analyzed.A first-level prevention model of empty-nester dementia was formed in two rounds.Results 30 expert questionnaires were sent out in the two rounds of consultation,and the recovery rate was 100%.All the questionnaires were valid,indicating that the experts were interested in the research and had a high enthusiasm to participate in it.The expert authority familiarity coefficient is 0.79,the judgment coefficient is 0.83 and the authority coefficient is 0.78.Cronbanch’s a coefficient is 0.83.After the first round of expert consultation,the importance score of 32 items ranged from 3.32 to 4.86,and the coefficient of variation ranged from 2.48%to 5.06%.The indexes with mean value of importance>3.5 and coefficient of variation<20%were taken as the criteria for entering the second round of consultation.After two rounds of consultation,4 items were added and 7 items were modified.After the second expert’s inquiry,the importance score of 36 items was 3.42-4.86,and the coefficient of variation was 2.96%-6.64%.Conclusion Through two rounds of expert consultation,the index system of dementia prevention for the empty nest elderly in community was initially established to provide reference for community intervention for the empty nest elderly.After that,the index system needs to be verified and further improved.
